About this file

This is a Past Performance Questionnaire template for reference laboratory testing services. The form is designed to be completed by customers or evaluators with direct experience working with an offeror to assess the contractor's performance record as part of evaluation criteria for a Reference for Quote (RFQ) solicitation.

Part Two requests general information including the offeror's name and address, customer organization details, customer point of contact information, contract number, contract value, period of performance, contract type, complexity of work (difficult or routine), and a brief description of contract requirements. Part Three contains a rating matrix requiring evaluators to assess the offeror's performance across ten specific factors using four confidence levels: Superior, Satisfactory, Unsatisfactory, or Neutral. The rated factors include Transmission/Connectivity to IT systems, Quality of IT Support, Startup Time from contract award to contract start date, Test Menus, Turn Around Times, Notification of Specimen Evaluation Viability, Metrics for Critical Results, Specimen Pick-Up Time, and Continuing Education Materials (PACE, CME, and CE). Part Three also includes narrative questions regarding contract terminations for default or convenience, contractor strengths and weaknesses, and whether the evaluator would award another contract to the contractor. Part Four contains an evaluator certification statement. Completed forms should be returned directly to the Contracting Officer at Morgan.Stein@va.gov. The form is administered by the Department of Veterans Affairs (VA).
