About this file
This is a Past Performance Questionnaire template issued by the U.S. Department of Veterans Affairs, Network Contracting Office (NCO) 02, for evaluating contractor performance on previous contracts. The questionnaire is designed to assess past performance as part of the evaluation criteria for solicitation 36C2422600504.
The questionnaire requires respondents to evaluate contractors across four primary performance categories using a six-point rating scale (Exceptional, Very Good, Satisfactory, Neutral, Marginal, or Unsatisfactory): Quality of Service (5 items), Timeliness of Performance (4 items), Business Relations (10 items), and Management of Key Personnel (5 items). Completed questionnaires must be returned no later than the solicitation's due date via email to Tiffany Vazquez-Simon, Contract Specialist, at tiffany.vazquez-simon@va.gov. The form also requires respondents to provide contractor information (name, address, point of contact) and contract information (contract number, type, award date, period of performance, total dollar amount, and scope description) to determine relevancy and recency. Handwritten remarks are encouraged, particularly when ratings of Exceptional, Marginal, or Unsatisfactory are assigned. Respondents must also provide a final recommendation regarding the contractor's suitability for similar future requirements.
