About this file
The document is a Past Performance form for a federal contract opportunity related to Beverly National Cemetery. The form requires bidders to provide detailed references from at least five recent customers, prioritized by National Cemetery Administration (NCA) facilities, VA facilities, other federal agencies, and commercial customers. A minimum of three references must be submitted, and the NCA may contact these references to evaluate the bidder's performance across four key areas: capability, contract compliance, cooperation, and customer satisfaction.
The form instructs bidders to document specific contract details for each reference, including contract number, project title, project description, total contract value, period of performance, customer name, and point of contact information. Past performance references will be reviewed for quality performance, contract timeliness, cost control, and business relations, using the Contractor Review Performance System (CPARS). The document notes that past performance questionnaires may be sent to listed firms, and the government will also review CPARS Performance Reports to evaluate the bidder's historical performance.
