About this file

This is a Parking Restriping Checklist and Evaluation of Service Tour form used by the Department of Defense, Veterans and Emergency Management - Military Bureau to assess contractor performance on parking lot restriping and crack filling projects for the Bangor Area Maine Army National Guard (MEARNG) facility. The contractor is responsible for restriping all parking spaces including standard spaces, electric vehicle charging spaces, handicap accessible spaces, motorcycle spaces, and no-parking areas, as well as restriping walkways and covering unused lines with blank paint. The contractor must provide all materials necessary to complete the work and perform all tasks during scheduled times. Evaluation of Service Tours must be conducted within 72 hours of the contractor's notification that work is complete, with the tour confirmed one week in advance. The contractor is required to contact the Program Administrator or designee within 10 days of contract execution to establish a tentative schedule for the work.

The contractor will be given two weeks from the date of the Evaluation Tour to correct any deficiencies identified during the inspection. Failure to complete corrections within this timeframe may result in contract termination with thirty days' notice, and poor performance may be considered in the evaluation of subsequent bids submitted by the contractor. The contractor has fifteen calendar days from the date of termination notification to appeal any contract termination decision to the Director of the Division of Procurement Services. If the contractor fails to contact the Department designee as required or does not attend the scheduled Evaluation of Service Tour, the level of service will be documented by the Department, and the contractor will not be able to dispute the evaluation. The evaluation checklist includes yes/no ratings for general tasks such as material provision, proper space sizing and coloring, symbol usage, crack filling with appropriate materials, and adherence to scheduled work times.
