About this file

This is a paperwork schedule document issued by the City of Toledo, Ohio, outlining a streamlined approach to collecting documentation from bidders for contracts advertised on or after March 13, 2026. The document establishes a three-tiered submission structure to reduce redundancy in the bidding process. City paperwork—including vendor registration forms, business tax registration forms, child support affidavits, and vendor utility information—must be submitted once by new vendors and retained for the duration of all contracts with the City. Annual paperwork, which includes W-9 forms, certificates of insurance, workers' compensation certificates, EEO compliance documentation, debarment certifications, and living wage affidavits (updated by February 1st), must also be submitted once annually for all bids submitted in 2026. Project paperwork, consisting of primary/subcontractor agreements, acknowledgements of terms and conditions, living wage contract addendums, and best bid criteria when applicable, must accompany each individual bid submission.

Vendors submitting their first bid on or after March 13, 2026, must provide all three categories of documentation. Subsequent bids from the same vendor require only project paperwork submissions. The City emphasizes that vendors are responsible for notifying the Division of Purchases and Supplies and updating documentation if any information changes. For questions regarding paperwork submission requirements, vendors should contact the city at pursupplies@toledo.oh.gov. This streamlined approach aims to reduce administrative burden on bidders while maintaining necessary documentation and compliance standards for the City of Toledo's procurement process.
