About this file

This is a draft quote sheet template for the MSPV (Medical Surgical Products and Supplies) Alternative Sourcing solicitation issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 1 (VISN 1). The document serves as a pricing template for vendors to submit quotations in response to solicitation number 36C24126Q0470.

The quote sheet template is designed to collect standardized pricing information from offerors bidding on medical surgical products and supplies through an alternative sourcing approach. As a template document, it establishes the format and structure that vendors must follow when submitting their pricing proposals, ensuring consistency and comparability across all received quotes. The specific products, quantities, delivery terms, pricing terms, set-asides, and response deadlines would be detailed in the associated solicitation documents and the Combined Synopsis/Solicitation Notice referenced as 36C24126Q0470_2.

Responsible party: VHA Office of Acquisitions Contracting Officers

Step 1 - Complete Solicitation Information:

These cells are pre-populated by the Contracting Officer responsible for the solicitation. The template is pre-populated based on selections made by the CO on the Quote Sheet Upload page.
