Justification for Single Source Awards IAW FAR 13.106‐1 For

Over Micro‐Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C249‐25‐AP‐1138

1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office (NCO) 09, located at 1639 Medical Center Parkway, Suite 204, Murfreesboro, TN 37129 in support of Tennessee Valley Healthcare System (TVHS), Alvin C. York Veterans Affairs Medical Center (VAMC) , 3400 Lebanon Pike, Murfreesboro, TN 37129.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Vericor Medical emergency kits provide the necessary components for medical staff to care for casualties when they arrive on the scene during a Mass Casualty Incident at each level of care. Each kit and related component also act as a storage device for all supplies and equipment related to patient care and examination and staff personal protection. TVHS maintains medical supplies and equipment for two Dual Use Vehicles (DUVs) and an Alternate Care Site (ACS). These sets enable the VA to transport patients in the DUVs between fixed and remote locations, while the ACS is designed to provide basic emergency care outside the hospital during an emergency situation or airfield operations.

Estimated amount: $138,108.45

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The current ACS kits and DUV kits were put together by Vericor, LLC. Upkeep of critical data (lot #, serial #, expiration date, and medical supply location), the introduction of new inventory items, and regular replenishment data in digital and printed form, which deems it necessary for the acquisition of the same replenishment and supply service for the purpose of meeting compliance. The medical facility cannot meet the requirements or effectively create an emergency management plan without knowing the exact number of supplies in its inventory. Since the current supplies are clearly marked with lot numbers, expiration, and serial data for all future replenishment activities, this request is necessary in order to meet compliance and continue supporting the current actionable response system. The replenishment supplies must be inventoried yearly and re‐stocked with identical items from Vericor, LLC. To replace the entire system to another manufacturer, would require an additional expenditure to replace all the kits, supplies, equipment and the additional cost of retraining staff in the use of the new device across TVHS.

4. Description of market research conducted and results or statement why it was not conducted:

Market research efforts include queries on Federal Procurement Data System (FPDS), General Services Administration (GSA) eLibrary, and System for Award Management (SAM) which rendered no compatible hospital triage and treatment modules. Market research has been conducted to find

Revision: 01 Effective Date: 11/15/2024 Page 2 of 2 Acquisition ID#: 36C249‐25‐AP‐1138 similar products but the kits that we have found are for “in the field” use and do not have the comprehensive supplies and equipment needed for a large‐scale event.

The small business vendor, Vericor, LLC; is the only vendor to provide the customizable training, replenishment services, and inventory manuals necessary for TVHS hospital to prepare for, respond to, and recover from a sudden crisis. This requirement will be awarded in accordance with (IAW) FAR (Federal Acquisition Regulation (FAR) 13.106‐1(b)(1)(ii).

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106‐1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
