This is a Statement of Work (SOW) for the Patient Elopement and Wander Management System replacement and expansion project at the Dallas VA Medical Center. The project requires replacement of the existing system at Dallas Community Living Center (Building 60) and Bonham Community Living Center (Building 29), with expansion into three acute care areas (5C400, 6C400, 7C200) at Dallas VA Medical Center. The contractor must provide enterprise software virtualized on a facility-supplied server, client workstations with proximity card readers and touchscreen monitors at all nurses' stations, door and zone control with magnetic locks capable of locking when monitored patients are detected, and patient wrist band transmitters with GEO tracking capability. The system must integrate with existing fire alarm and security systems, display alarms on a CAD drawing-based interface showing patient information and location, store patient roaming history and zone status, and support customizable web-based reports.

The contractor is responsible for complete equipment installation, networking, electrical and data work, limited dust containment/infection control measures, and training for nursing and biomedical staff across all shifts. The period of performance runs from August 1, 2026 through July 31, 2027, with work scheduled Monday-Friday, 8:00 AM to 5:00 PM CST. All installation activities must comply with the facility's Infection Control Risk Assessment (ICRA) Class III dust containment requirements and Interim Life Safety Measures (ILSM). The contractor must obtain daily check-in with Biomedical Engineering, comply with extensive VA information security requirements including HIPAA privacy provisions and Rules of Behavior, and provide detailed field service reports documenting equipment details, work performed, test results, and parts replaced. A mandatory site visit is required before bid acceptance, and the system must be capable of supporting future expansion into asset and equipment tracking and staff duress functionality. The place of performance is Dallas VA Medical Center, 4500 S. Lancaster Road, Dallas, Texas 75216.

Statement of Work (SOW) Contract Title Patient Elopement and Wander Management System (Replacement and Addition) Background The existing Patient Elopement and Wander Management System at the Dallas and Bonham Community Living Centers (CLC) require replacement due to age and technological deficiencies. This effort will also expand elopement monitoring coverage into select acute care areas within the Dallas VA Medical Center (5C400, 6C400, 7C200) Scope The Dallas VA Medical Center, part of the VA North Texas Healthcare System, requires the purchase and installation of a Patient Elopement and Wander Management System. The system will replace the existing system in the Dallas CLC (Building 60) and Bonham CLC (Building 29), and will expand into acute care areas 5C400, 6C400, and 7C200 in Dallas. The system will encompass the areas and specifics are detailed below.

Software and Networking Enterprise software must be virtualized onto a facility-supplied server located at the facility's data center, communicating via the Hospital's Local Area Network (LAN). Facility maps will be installed, and all system devices will be programmed into the system. The vendor will provide remote support access software for connectivity to technical support staff. The facility will be responsible for providing a working internet connection and IP address at the server location.

Client Workstations and Monitoring Displays Client workstations with proximity card readers for user login and touchscreen monitors must be installed and configured for enterprise use, communicating on the Hospital's LAN. A Staff Alert Panel (SAP) or similar may be installed at each coverage area as an analog backup for the computer system; in the event of a computer failure, the SAP will notify staff of any door and zone alarms. Workstations must be installed at each nurses' station within the coverage areas. All workstations must be networked to one another and to the server via an IT-provided VLAN switch.

Door and Zone Control Designated door zones must be capable of locking down using magnetic locks when a monitored patient is detected and the doors are closed. Not all doors require locking capability; the specific doors requiring lock integration will be identified during walkthroughs. Zone controllers must alarm and, where applicable, close and lock doors when a monitored patient enters the zone. All designated locked doors must remain locked until staff relocates the patient. Each door with locking capability may include a keypad or touchscreen display on both the protected and unprotected sides to clear alarms or grant emergency access via individual staff PIN codes. Virtual hallway door zones with motion control will also be monitored. Where conduit is required, procurement and installation of conduit will be the responsibility of the facility; the vendor will supply drawings to assist at no additional cost.

Alarm Identification and Reporting The system must store patient roaming history and zone status. Alarms must be displayed on a CAD drawing-based interface showing the patient's name, photo, and any associated warning messages. Upon alarm, the system must identify the zone or area where the patient triggered the alarm. The system must support customizable, web-based reports printable from a standard printer. The system must also provide GEO tracking capability for monitored patients.

Transmitters Patient wander transmitters with patient ID and/or banding must be supplied for use across all monitored units.

Integrations The system must integrate with the following:

· Existing fire alarm system (all designated locked doors must automatically unlock upon fire alarm activation). All fire release integration, permits, and associated costs are the responsibility of the facility.

· Existing security systems

Training The vendor must provide in-service instruction and go-live support for clinical personnel across all covered areas. The time and date for training will be established upon receipt of a purchase order.

Installation The vendor is responsible for all equipment installation and site preparation, including any electrical and data work required to make the system fully operational. Installation of all required networking equipment is included in this scope. The vendor will provide limited Dust Containment/Infection Control measures throughout the installation. Vendors will need to move ceiling tiles if necessary and vacuum the work areas upon completion.

· Infection Control Risk Assessment (ICRA) – All installation activities must comply with the facility's Infection Control Risk Assessment (ICRA) requirements. Prior to beginning work, the vendor must coordinate with the facility's Infection Control team to determine the applicable ICRA category for each work area and implement all required containment and protective measures accordingly. Work must not proceed in any area until ICRA requirements have been reviewed and approved by the facility's Infection Control representative. All installation activities must adhere to Class III dust containment requirements in accordance with the facility's Infection Control Risk Assessment (ICRA).

· Fire-Retardant Plywood – Where backboards are required for mounting head-end equipment, the vendor may reuse existing fire-retardant painted plywood provided it is structurally sound and in good condition. Reuse of existing plywood is permitted as a means of maintaining the room's existing fire rating. Any plywood determined to be damaged, deteriorated, or otherwise unsuitable for reuse must be replaced with new plywood that meets the same fire-retardant rating as the original material. The vendor must document the condition of any reused plywood and make that documentation available to the COR upon request.

Equipment Specifics Equipment needed for these areas will depend on coverage required by the site. Included in this requirement are floor maps of the areas where the installations will be performed.

· Patient Wrist Bands – Quantity dependent on rooms identified as high-risk areas

· Server Bay Installation: Install rack mountable 1U server in provided 19” rack space and connect to provided ports to switch.

· Nursing Staff Training: Training for the Nursing Staff will be provided for each shift.

· Old Equipment Removal: When new equipment has been installed and tested, the old equipment can be removed and turned over to Nursing Staff for proper disposal.

· Deliverables: Deliverables will be received by the warehouse and delivered to a designated space until supplies are needed for installation.

· All head-end equipment and computer systems provided under this contract must be connected to a vendor-supplied and installed Uninterruptible Power Supply (UPS) of sufficient capacity to maintain system operation during a loss of primary power.
