About this file

This file outlines the Personal Identity Verification (PIV) process for contracted employees at a VA facility. The process requires proof of company background check, specified immunizations (detailed in SOW), and drug screening within 30 days, which must be verified by the Contracting Officer's Representative (COR). Employees must complete three forms and submit them separately via email along with their email address and marital status.

After form verification, employees enter the VA sponsorship system and undergo fingerprinting, typically conducted in mass sessions on Saturdays. Employees can begin work after fingerprinting and PIV office clearance, with the understanding that unfavorable background check results will result in immediate removal from campus. The document relates to solicitation 36C26325Q0386 for Housekeeping Aide Services at VA Nebraska Western Iowa Health Care System, which is a Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside with a base period from March 2, 2025, to March 1, 2026, plus one one-year option period.

-COR will need verifica on that each of those have been completed for each employee before the next steps. (Email will suffice from company).

Once those items have been verified and sent to COR, a ached are the 3 forms that need to be filled out thoroughly. If anyone answers yes to ques ons 9-15 those have to be explained in full detail in block 16. Those forms need to be completed carefully and returned to me.

When those forms are sent back to the COR, its needs to be in an email with all 3 a achments, a ached separately and also needed is a valid email for the employee and their current marital status.

“EXAMPLE EMAIL: Jane Doe has passed the required drug tes ng and immuniza on check; therefore, I have a ached the OF306, Self Cer fica on Form, and VA Privacy training.

Email Address:

Marital status:

Shi : 2nd Shi ”

Once the COR receives the forms, they will look over and ensure everything is filled out correct. If the forms are not filled out correctly they will be returned for correc ons and they will most likely not make the mass fingerprint and will have to set an appt.

We(VA) will put them in the system for sponsorship. A er that they can be scheduled for fingerprints. (Want to try and do a mass fingerprint session on a Saturday). NOTE: there will be certain required documents they will need to bring for fingerprin ng.

Once they are sponsored, they will need to schedule their appt with the PIV office on the website.

Once they have been fingerprinted and the appropriate documents have been sent to WMC-and the employee is cleared by the PIV office the employee can start work with the understanding if the fingerprints/background check comes back unfavorable they will be escorted off campus immediately.

PLEASE NOTE: They will have more requirements to do on a cpu from WMC as well so they need to check their email.
