About this file

This is an insurance requirements exhibit document from the Pennsylvania Department of Transportation (PennDOT) for contract OS-52. The exhibit establishes mandatory insurance coverage specifications that the awarded contractor must purchase and maintain throughout the entire contract term and any renewals or extensions. Insurance must be in effect prior to commencing work or entering onto Department of Transportation property. The contractor is required to obtain workers' compensation insurance covering all employees as mandated by Commonwealth law, and comprehensive general liability insurance with minimum coverage limits of $250,000 per person and $1,000,000 per occurrence for bodily injury (including death) and property damage, along with automobile liability insurance where applicable. All coverage must be occurrence-based, with the Commonwealth or Department named as an additional insured party.

The contractor must provide current certificates of insurance to PennDOT demonstrating the required coverages and must include policy provisions requiring at least thirty days' prior written notice before any cancellation or modification of coverage, except in cases of nonpayment where fifteen days' notice applies. If certificate information is insufficient, supporting policy documents confirming the Commonwealth's additional insured status and cancellation notice provisions must accompany the certificates. The contractor bears full responsibility for notifying the Department of any insurance cancellation and must maintain continuous coverage throughout the contract period; no work may proceed or Department property may be accessed during any lapse in required insurance coverage.
