About this file
This document is a Hazard Assessment Review Form for the Oregon Health Authority (OHA) regarding the Toxic Free Kids Act. The form is to be used by contractors reviewing a hazard assessment (HA) conducted using an alternative methodology approved by OHA, rather than the required GreenScreen methodology. OHA will provide the approved alternative methodology and a review form specific to that methodology. The form includes fields for the contractor to provide information about the manufacturer submitting the HA, the firm contracting with OHA, and the reviewer's contact details. The document outlines the requirements for HAs under the Toxic Free Kids Act, including when they are required and the approved methodologies.
The related state and local contract opportunity is a Request for Application for the review of waiver and hazard assessment requests. This is an opportunity for firms to contract with OHA to review these submissions, which are required under the Toxic Free Kids Act rules for children's products.

Directions to Contractor Reviewing a Hazard Assessment Conducted with an Alternative Methadology
Under rules for Oregon's Toxic Free Kids Act, hazard assessments (HA) shall be conducted for: (a) when a manufacturer intends to substitute HPCCCH with another chemical in specific children's products (OAR 333-016-3020) or (b) for an Alternatives Assessment (OAR 333-016-3060) submitted in support of a waiver from the requirement to remove or substitute a HPCCCH in specific children's products (OAR 333-016-3010).
In both cases, HA are to be conducted by a Licensed GreenScreen Profiler (LGSP) using GreenScreen methodology (OAR 333-016-3030(1)(A)). If part of an Alternatives Assessment, HAs conducted by a LGSP are to be treated “as is” by Contractors with the conclusions made by the LGSP not contested. Such HAs are sent with the other modules of an Alternatives Assesment to inform the Contractor's review of those modules.
Manufacturers may request that OHA approve an alternative methodology (OAR 333-016-3030(4)). Such requests must be made 90 business days before the resulting HA is submitted to OHA. If a contractor is asked to review a HA conducted with an approved alternative methodology, OHA will provide both the approved methodology itself as well as a review form specific to that approved methodology. To be approved, alternative methodologies must meet the requirements in OAR 333-016-3030(4) and (5).
