About this file
This document is an Application Form for Request for Application (RFA) OHA-5171 issued by the Oregon Health Authority (OHA) for Quantitative Exposure Assessment, Hazard Assessment, or Alternatives Assessment Review Services. The RFA seeks applicants who can provide these assessment review services, focusing on High Priority Chemicals of Concern for Children's Health under the Oregon Toxic-Free Kids Act. Applicants must meet specific requirements, including having relevant experience, understanding the Act and related rules, and demonstrating their qualifications and capacity to perform the work. They must also provide evidence of commercial general liability and professional liability insurance. The application form requires applicants to submit information about their organization, the specific review services they are applying for, and various attestations related to their qualifications and approach.
The related state and local contract opportunity is for the Review of Waiver and Hazard Assessment Requests, indicating that the selected applicants will be responsible for conducting these specialized reviews on behalf of the OHA. The application process has specific deadlines, and selected applicants will enter into an agreement with the OHA to provide the required services.
