Re: Report of Existing Materials Testing Harold Oliver Middle School – Existing Materials Testing Addendum 1
15840 SE Taylor Road, Portland, Oregon 97233 PSI Project # 07023229
Dear Mr. Bodell:
Professional Service Industries, Inc., (PSI) an Intertek company, is pleased to submit this addendum report of construction materials testing services for the above referenced project. Please reference our original report dated May 28, 2024 for more information.
Scope of Work
PSI performed existing materials testing in general accordance with the testing requirements requested by ZCS Engineering and Architecture as shown in their Material Testing Plan V3. Work consisted of GPR survey, existing material sampling and testing of concrete, reinforcing steel, and brick and CMU masonry. We understand the requested services are to determine the material properties in advance of a seismic retrofit.
Specific Services Included:
• GPR mapping of existing reinforcing steel at the concrete diaphragm and CMU wall locations.
• Two core samples were extracted from the concrete diaphragm and tested in general accordance with ASTM C39 and C42.
• One CMU masonry prism sample was extracted and tested in general accordance with ASTM C1532 and C1314.
• Removal and testing of two reinforcing steel samples from the concrete deck location in general accordance with ASTM A370. Reinforcing steel sample locations were spliced with couplers.
• Performed Flexural Bond Strength Testing of exterior brick masonry walls at five locations in general accordance with ASTM C1072 modified for in situ testing using the bond wrench method.

PSI Project No. 07023229 11/1/2024
• Sampling locations at concrete deck and the CMU wall were patched with US Spec concrete products.
• Test locations at exterior brick walls were patched with brick and mortar.
• Document and report visible conditions and nature of materials and connections in the aforementioned locations.
FINDINGS
Below is a general summary of findings for the concrete and CMU masonry and brick masonry structural elements. The sampling location drawings and photographs attached as referenced contain additional location specific information.
Mezzanine Concrete Diaphragm
As requested, the existing mezzanine concrete diaphragm was scanned at the location shown on the attached sampling plan. GPR results indicated existing concrete deck was approximately 5-1/2 inches thick and reinforced with a single mat spaced approximately 12” on center each way. Reinforcing steel exposed at sample locations consisted of #5 deformed bars at a depth of 4 to 4-1/2” below top of slab. PSI collected two cores for compressive strength testing and reinforcing steel samples for tensile strength testing. See attached test results.
Interior CMU Wall
As requested, an existing CMU wall was scanned at the location shown on the attached sampling plan. Based on GPR results, CMU wall was nominally 8-inches thick and appeared to be partially grouted in cells containing vertical reinforcing. Vertical reinforcing was exposed at one location by chipping away face shell and grout and was visually observed to consist of a single #4 deformed bar.
Horizontal joint reinforcement was also encountered at the CMU sample location. PSI collected a hollow masonry prism sample for compressive strength testing. See attached test results.
Exterior Brick Walls
As requested, PSI performed flexural bond strength testing of the exterior brick walls using bond wrench for in situ testing at the locations shown on the attached sampling plan. See attached results for the requested in situ bond wrench flexural strength testing.
Visual Appearance
Visual appearance of existing concrete mezzanine diaphragm and interior CMU wall and exterior brick walls appeared normal at the test locations, with exception of cracking observed at the mezzanine floor concrete diaphragm as shown on the attached sample description photos.
LIMITATIONS
Interpretations are based on standard test methods, specification and our experience evaluating masonry and reinforcing steel materials. Services performed for this project have been conducted with the level of care and skill ordinarily exercised by members of the profession currently practicing in this area under similar budget and time constraints. The report was prepared for the sole use of the Centennial District and ZCS. Reports may not be reproduced, except in full without the written permission by PSI. No warranty, expressed or implied, is made.
