About this file
This is an Office Automation Form from the City of Philadelphia (PHLContracts) that establishes installation and configuration requirements for office equipment procurement. The form specifies that vendors must deliver products inside to a loading dock, provide installation and configuration services, and submit product literature upon request. The contract requires extended warranty coverage with minimum periods of five years for servers and IT appliances, four years for PCs and laptops, three years for tablets, and the original equipment manufacturer's warranty for peripherals including printers, copiers, and multifunction devices. These warranty periods commence after a 30 to 120-day acceptance period. The form indicates that vendors must offer next-day on-site maintenance response within 24 hours, including all parts and labor, with payment due after equipment delivery and City acceptance.
The form permits vendors to provide either a 3-year or 5-year maintenance option and requires vendors to specify whether instruction and training services are requested. Vendors must indicate their acceptance of refurbished equipment as part of their quote submission. The document establishes checkboxes for vendors and the City to confirm product specifications, configuration platforms, training provision, warranty selections, and maintenance term preferences. All city departments requesting office automation equipment through PHLContracts must complete this form, and vendors are required to review all requirements prior to submitting their quotes.
