This document is an Invitation for Bid (IFB) "Offer to the State" form submitted to the State of Illinois for snow removal services at facility J0951, located at 9511 Harrison Street, Des Plaines, Illinois 60016. The solicitation is designated as a Small Business Set-Aside (SBSA) opportunity. The form serves as the bidder's formal submission in response to the Illinois procurement solicitation, requiring bidders to confirm compliance with all solicitation requirements and submit required documentation as outlined in Section A.1 of the solicitation document.
