About this file
This is an Invitation for Bid offer submission form for the State of Illinois, specifically for IDOT's OSHA/ANSI Testing and Inspection Service for Aerial Lifts and Cranes (Solicitation B-51979). The document serves as the formal bid submission template that vendors must complete and return to be considered for the contract award. The solicitation seeks a vendor to provide OSHA/ANSI inspection services for lifting and hoisting equipment. The form requires bidders to confirm their understanding that failure to meet all requirements specified in the solicitation document section A.1 will result in disqualification.
The offer form includes sections for bidders to claim applicable preferences under the Illinois Procurement Code, including the Illinois Business Preference and Domestic Product Preference, with vendors required to submit supporting affidavits or product descriptions as applicable. The form also provides space for bidders to take exceptions to the Standard Terms and Conditions by submitting a redline version, which the State may accept or reject at its discretion. The State has established a Business Enterprise Program (BEP) goal of 3% for this solicitation. The document indicates that Standard Terms and Conditions will be incorporated into any resulting contract and are available on the Illinois CPO website. By signing this form, the bidder submits a formal offer to the State of Illinois in full compliance with the solicitation requirements.
