About this file
This is a Vendor Assessment Form for the State of Rhode Island Division of Purchases' open enrollment Master Price Agreement (MPA 611) for COVID-19 testing kits. The solicitation (OEV24004199) seeks qualified vendors to supply FDA-authorized rapid antigen test kits and laboratory-based PCR tests to help mitigate community transmission of COVID-19. The MPA will operate from May 1, 2024, through June 30, 2026, with an initial four-year term and options to renew for up to three additional 12-month periods. Vendors may submit proposals at any time during the open enrollment period, with evaluations conducted on a pass/fail basis. When agencies require testing kits, they will issue mini-bids to qualified vendors requesting detailed pricing quotes and project-specific requirements. The contract has statewide applicability and allows participation from political subdivisions, institutions of higher education, municipal and state schools, and quasi-public agencies.
Vendors must meet twelve mandatory criteria outlined in the assessment form, including the ability to procure FDA-authorized rapid antigen test kits, maintain two years of experience as a distributor or manufacturer of laboratory and point-of-care supplies, currently service government agencies, and deliver during normal business hours (8:30 AM to 4:30 PM EST). Additionally, vendors must demonstrate capacity to deliver orders up to 100,000 tests within 48 hours, supply standard orders of 50,000 to 200,000 tests within 14 days, and fulfill bulk orders of 200,000 to 700,000 tests within 45 days. Vendors must support standard shipping costs without expedited surcharges, accept Net 30 payment terms, maintain inventory within the continental United States, and provide palletized deliveries in 40" x 48" x 72"h size pallets. A 1% contract administrative fee will be withheld from vendor payments, and vendors must provide three government references with their pricing sheet. The solicitation emphasizes Minority Business Enterprise participation, requiring an MBE Participation Acknowledgment Form and ensuring at least one of three quotes comes from MBE/WBE/VBE certified vendors when available. Vendors must maintain $1 million in liability coverage, workers compensation insurance, and automobile liability coverage.
