About this file
This document is a Disclosure Statement from Events DC, a Washington, D.C. organization, requiring principals to disclose any past or present business, familial, or personal relationships with the organization's Board of Directors and Key Staff Members. The form provides a comprehensive list of board members, including Leif Dormsjo (Chairman), Cherrie Doggett (Treasurer), and various other members, as well as key staff members such as Angie M. Gates (President and CEO), Dr. Steven G. Johnson (Executive Vice President), and other senior leadership across different departments.
The disclosure statement outlines two potential submission scenarios: (1) if no relationships exist, a signed statement can be submitted, or (2) if relationships are present, the statement must detail the nature of those relationships. The form includes a certification statement where the principal attests to the full and accurate disclosure of any business, familial, or personal relationships with Events DC Board and Staff members. Principals are required to sign and date the document, providing their name and title, which serves as a formal acknowledgment of potential conflicts of interest or connections to the organization.
