About this file
This document is a Disclosure Statement for Events DC, a local government agency in Washington, D.C., that requires potential contractors or bidders to disclose any past or present business, familial, or personal relationships with its Board of Directors and key staff members. The statement provides a comprehensive list of Events DC's Board of Directors, including Leif Dormsjo as Chairman and members such as Cherrie Doggett, Nina Albert, and others, as well as a detailed roster of key staff members from the President and CEO Angie M. Gates to various Vice Presidents and Directors.
The disclosure form is structured to ensure transparency and potential conflict of interest reporting, with three main sections: A) a requirement for principals to disclose relationships, B) an option to submit a signed statement if no relationships exist, and C) a certification process for those with existing relationships. The document includes a signature section where individuals must attest to the accuracy of their disclosure, with spaces for name, date, and title. This type of disclosure is typically part of a procurement process to maintain ethical standards and prevent potential conflicts of interest in business interactions with the organization.
