About this file
This document is a Request for Grant Application (RFGA) issued by the Oregon Health Authority (OHA) Maternal and Child Health (MCH) Section for Infant Safe Sleep Campaign Community Mini Grants. The objective of this mini grant is to develop culturally specific, local safe sleep awareness campaigns targeting African American/Black families and/or Native American/Alaska Native families. Eligible organizations may receive up to $30,000 per grant to implement a community awareness campaign using safe sleep messages developed by the Oregon Safe Sleep Coalition. Applications are due by May 23, 2022, and grants are expected to start on July 1, 2022 and end on June 30, 2023.
The program budget and allowable expenses, including staff time, outreach materials, incentives, and childcare, are provided in the application. OHA intends to award up to five grants and will evaluate applications based on the organization's experience working with the target communities, prior experience engaging pregnant people and families with young infants, and the proposed approach to implementing the safe sleep campaign. The OHA MCH Section and Oregon Safe Sleep Coalition will use the lessons learned from the community campaigns to inform future safe sleep awareness efforts.

Request for Grant Application
Public Health -Maternal and Child Health (MCH)– Safe Sleep Mini Grants
In compliance with the Americans with Disabilities Act, this document is available in alternate formats such as Braille, large print, audio recordings, Web-based communications, and other electronic formats. To request an alternate format, please send an e-mail to dhs-oha.publicationrequest@state.or.us or call 503-378-3486 (voice) or 503-378-3523 (TTY) to arrange for the alternative format.
1. Objective
The objective of this mini grant is to develop culturally specific, local safe sleep awareness campaigns.
2. Introduction.
In Oregon, about 40 babies die in their sleep every year. Nationally and in Oregon, rates of Sudden Unexplained Infant Deaths (SUID) for American Indian/Alaska Native and non-Hispanic Black infants are more than twice those of non-Hispanic White infants.
Given the large and persistent disparities in SUID deaths by race/ethnicity, it is important to examine SUID through the lens of structural inequity and historical trauma.
Sleep related infant deaths can be prevented when parents, caregivers and childcare providers are supported in following recommended safe sleep practices. Having multiple settings share consistent safe sleep messages supports safe sleep practices by helping make them a community-wide norm and expectation. This helps parents, caregivers and childcare providers practice these behaviors, building a cultural context of support for safe sleep behaviors and removing cultural barriers.
OHA intends to award up-to five grant agreements. Eligible applicants may receive up-to $30,000 per grant.
3. Purpose
To develop and implement culturally responsive community awareness campaigns to promote safe sleep for infants. Community campaigns will use health messages developed by the Oregon Safe Sleep Coalition and be based on best practices in promoting safe sleep. The MCH Section and the Oregon Safe Sleep Coalition will use the lessons learned from these community campaigns to inform future safe sleep awareness campaigns.

4. Eligibility
The following organizations are eligible to apply:
● Any non-profit agency or other organization that provides culturally responsive services to the following communities of focus:
o African American/Black families o Native American/Alaska Native families
● Organizations with an interest in health equity
● Any federally recognized tribe or tribal-related organization
● All Local Public Health Authorities
● Organizations serving areas outside of the Portland metro area are strongly encouraged to apply.

5. Grant and Reporting Requirements
Grantees will develop and implement a culturally responsive community awareness campaign to promote infant safe sleep among African American/Black families and/or Native American/Alaska Native families. Specific approaches can be modified by grantees to be community specific. Community campaigns will include use of health messages developed by the Oregon Safe Sleep Coalition, be based on best practices in promoting safe sleep and focus on pregnant people and/or people caring for infants less than 6 months old (including childcare providers).
Grantees will identify the method for engagement that will work best for their community. Options include, but are not limited to:
● Community baby showers
● Tabling at community events
● Activities for Safe Sleep Awareness month in October
● Educating community members, including extended family, friends and local businesses—anyone who wants to support safe sleep messages and practices.
Throughout the grant period, grantees will participate in a monthly Safe Sleep Community of Practice virtual meeting to share best practices and promote collaborative learning among the grantees and the Oregon Safe Sleep Coalition.
The following reporting will be required during the term of the grant agreement:
● Written report with narrative describing process used, number of people reached, and lessons learned. The reporting template will be provided by the MCH Section (Due date: June 30, 2023). See Appendix B for more specifics about the required report.

The SPC will average the scores for each Proposal in a given round of competition (calculated by totaling the points awarded by each Evaluation Committee member and dividing by the number of members). The SPC will combine the average score for each Proposal with Proposer’s price score and reference score.
After any applicable preference has been applied, the SPC will determine the rank of each Proposal, with the highest score receiving the highest rank, and successive rank order determined by the next highest score.
Agency may, in Agency’s sole discretion, determine an apparent successful Proposer with no additional rounds of competition. If additional rounds are conducted, Agency will rank advancing Proposers at the conclusion of each subsequent round and may determine an apparent successful Proposer at any time during the solicitation process.
