INSTRUCTIONS FOR USE:
1. The contractor shall prepare and submit the Over and Above Work Request (OAWR) form to the PCO, via the TPOC, for authorization to proceed.
2. Describe on the form the needed repairs and necessary work that needs to be performed, as well as the estimated cost.
3. Provide as an attachment to the form a quote and a cost break-out in such detail to permit a thorough evaluation by the Government. The cost break-out may be included on, or provided in addition to, the quote. Details should include (as applicable):
· Type, quantity and cost of parts and/or materials
· Labor hours, cost per hour, and labor category
· Any other pertinent information impacting cost
4. The TPOC will review the OAWR and then forward the form and attachment(s) to the PCO with a recommendation of approval or disapproval.
5. The PCO will:
· Authorize the OAWR. In this event, a modification to the PO will be issued. Upon issuance of the modification, the contractor shall proceed with the repair or replacement effort.
OR
· Disapprove the OAWR. In this event, the OAWR will be returned to the contractor. A copy will be provided to the TPOC.
6. As part of the authorization process, a cost and price analysis will be conducted by the Government. Based on the findings, the quoted price will be accepted as proposed, or a Firm Fixed Price will be negotiated with the contractor. Once price agreement has been reached, the modification to add the necessary funding to the PO will be issued.
