About this file

This is an Office Automation Form A submission for the City of Philadelphia requesting the procurement of 10 units of j5create USB 3.0 Multi-Adapter Hubs. The form outlines installation and configuration requirements, with products to be delivered pre-configured and any additional configuration to occur at the City's location. Instruction and training by the vendor are requested as part of this procurement. The contract specifies that all deliveries must be inside deliveries to a loading dock, with the vendor responsible for providing cost for installation and configuration unless otherwise directed. The vendor is required to submit product literature at the City's request.

The procurement includes mandatory warranty coverage with a minimum five-year warranty period for servers and IT appliances, four years for PCs and laptops, and three years for tablets, with original equipment manufacturer warranty applying to peripherals such as printers, copiers, and multifunction devices. On-site maintenance is required with next-day response capability (within 24 hours) including all parts and labor, with payment due after equipment delivery and acceptance by the City. The City offers a choice between a three-year or five-year maintenance option. The form indicates that refurbished equipment is not acceptable, and vendor selection criteria include completion of all requirements specified in the Office Automation Form A prior to quote submission through PHLContracts.
