About this file
This is an E-Verify Subcontractor Affidavit required by the City of Norcross, Georgia, pursuant to O.C.G.A. 13-10-91. The document establishes compliance requirements for subcontractors performing services under city contracts. Subcontractors must verify their registration with and authorization to use the federal E-Verify work authorization program prior to commencing work and must maintain this authorization throughout the contract period. The affidavit requires subcontractors to provide their federal work authorization user identification number and date of authorization.
The affidavit further obligates subcontractors to ensure that any sub-subcontractors engaged for physical performance of services also submit completed affidavits demonstrating E-Verify compliance. Subcontractors must forward notice of receipt of sub-subcontractor affidavits to the primary contractor within five business days. The document must be executed under penalty of perjury and notarized, with required signature blocks for both the authorized officer or agent of the subcontractor and the notary public. This requirement applies to all subcontractors and sub-subcontractors working on City of Norcross contracts and represents a mandatory employment authorization verification condition.
