About this file

This is a grant application document for the Positive Adolescent and Childhood Experiences to Prevent Substance Use (PACES) Initiative issued by the Massachusetts Department of Public Health under Notice of Intent Waiver 26138. The application is designed for municipal applicants, operating individually or through multi-municipal/county consortiums, to participate in a substance use prevention program targeting youth ages 5-14. Applicants must identify their proposed geographic service area, identify 1-3 community-based organization (CBO) partners with demonstrated experience serving youth and families, and document prior implementation experience in areas such as peer and family support relationships, safe environments, youth engagement and leadership, social-emotional development, parent/caregiver engagement, and strength-based programming frameworks. The application requires submission of a completed narrative response (limited to two pages for questions 2 and 4), completed checklists documenting relevant experience, and a signed letter of commitment from municipal leadership (Mayor, Town Manager, or Health Director). The submission deadline is July 3, 2026 at 4:00 PM to ryan.h.naismith@mass.gov.

Applicants must commit to participation in an initial three-month assessment phase, engagement of at least one local CBO partner throughout the assessment and implementation process, support for local health data collection and evaluation, and continued involvement in implementation and evaluation activities over the full five-year grant period. The application requires documentation of youth health survey data collected since 2021 for grades 6-12, including substance use prevalence data, protective and risk factors, and social-emotional measures, with data disaggregated by middle school (grades 6-8) and high school (grades 9-12) populations. If no recent surveys exist, applicants must describe the feasibility of implementing baseline data collection at project launch and biennially throughout the grant term. Applicants must also attach copies of the most recent survey instruments and results. No specific funding amounts or award dates are identified in the application document.
