About this file

This is a Traffic Control/Roadway Work Permit form issued by the New Mexico Department of Transportation (NMDOT) for contractors conducting work on state roadways. The permit requires contractors to submit detailed information about their project scope, including work location by route and mile post, road classification, existing and proposed speed limits, working duration with specific start and end dates, and the purpose of work such as roadway construction, utility work, drainage, or signing and striping. A Traffic Control Plan (TCP) must be enclosed with the permit submission, and the permit will not be processed without this documentation. The form accommodates various road configurations ranging from two-lane to eight-lane roads, both divided and undivided, and allows specification of work direction relative to cardinal directions.

Approval of the permit is conditional upon the contractor's compliance with the Manual on Uniform Traffic Control Devices (MUTCD) standards and acceptance of indemnification requirements protecting NMDOT and its employees from liability arising from the contractor's negligent acts. The contractor must provide NMDOT with a certified copy of insurance documentation listing NMDOT as an additional named insured with primary coverage over other insurance policies. The permit requires coordination between the contractor, traffic control firm, and the certified traffic control supervisor, with formal approval and submission to the District Public Information Officer. Additional conditions may be attached to individual permits, and contractors must contact the District Office to confirm actual start dates before commencing work.
