About this file
This document is an NDAA (National Defense Authorization Act) Compliance Certification form from the U.S. Department of State, specifically Attachment 1 related to representations regarding covered telecommunications equipment and services. The form requires vendors to disclose whether they will provide or currently use covered telecommunications equipment or services to the government, as mandated by Public Law 115-232 (John S. McCain NDAA for Fiscal Year 2019). Vendors must complete the representation, indicating their compliance status in two key areas: (1) whether they will provide covered telecommunications equipment/services in contract performance, and (2) whether they currently use such equipment or services in their systems. The form must be signed by an authorized representative and returned to the contract specialist, serving as a critical compliance mechanism for federal procurement actions.
