About this file

This is a Past Performance Questionnaire (PPQ) template for RFP NAFNDW-25-R-2002 related to child care spaces in the Washington DC area. The questionnaire is designed to evaluate contractors' previous performance across seven key factors: quality of work, personnel, subcontractor management, business relations, timeliness of performance, customer satisfaction, and cost/budget control.

The PPQ includes a six-level rating system ranging from Excellent to Unsatisfactory, with detailed definitions for each rating level. An Excellent rating requires very current and relevant performance with no problems, while an Unsatisfactory rating indicates failure to meet most contractual requirements with serious unresolved problems. The form allows for both numerical ratings and written comments for each evaluation factor. Previous PPQs may be used if Block 16's date is within one year of the RFP's original Block 9 offer end date. The questionnaire collects basic contract information including contractor details, contract type (Firm Fixed Price, Cost Reimbursement, or Other), period of performance, dollar value, and complexity level (Difficult or Routine).
