2. Identify if you are submitting a response to this sources sought under a formal Mentor Protégé agreement and, if so, provide evidence of such an agreement.
3. Identify if you are a subsidiary of another company.
a. Provide the related company’s name and the type of relationship with that company (e.g., wholly owned subsidiary, minor partner, etc.).
b. Describe the types and level of expertise and support that the related company may provide.
c. Describe the circumstances under which the related company shall provide support (e.g., projects in excess of a certain dollar amount, at the discretion of the program manager, etc.).
4. Identify all primary tasks (listed above) your firm has the capability to perform without subcontracting to other firms.
5. Where does your firm have the capability to perform these tasks?
6. Provide examples of projects performed as the prime contractor within the last five years of similar size, scope, and complexity as the work indicated above. Provide project specifics (scope, dollar value, location, references and contact information).
7. What is the maximum number of task orders under an Indefinite Delivery Indefinite Quantity (IDIQ) contract your firm has managed simultaneously as the prime contractor? Specify if these task orders were under a single contract or multiple contracts.
8. In accordance with FAR 52.219-14, demonstrate how your firm would perform at least 50% of the work without subcontracting to other firms.
9. List the number and type of licensed professionals in your firm (i.e., PSM, Architect, Engineer, GISP, etc.). If you are proposing to utilize a teaming arrangement, identify the number and type of professionals in each firm.
