STATEMENT OF WORK

Repair Manhole Covers Going Up Laguna Peak

1 General Intention and Scope of Work

The project will address the inadequate conditions of existing manhole concrete enclosures and steel plate covers. These manholes support various communication cables, connecting NAS Point Mugu infrastructures and Laguna Peak communication equipment assets. All six manhole concrete enclosures have different degree of deterioration, showing settlements, cracks and chips, which will be repaired through this project. The existing steel plate covers will require newly fabricated aluminum steel plate assembly, allowing one person to open and close.

1.1 Statement of Work

The contractor shall furnish all labor, management, supervision, tools, materials, equipment, incidental engineering, and transportation necessary to perform all ordered work. All work shall be performed on Regular Working Hours.

1.1.1 Demolition Work

a. Remove and dispose of all existing steel plate manhole cover plates and associated hardware.

b. Selective demolition of manhole concrete enclosures as a part of repair process.

1.1.2 New Construction

a. Field measurement shall be performed to verify the exact dimensions.

b. Make necessary repairs to each concrete manhole enclosures, addressing the settlement, cracks and chips to meet the Non-traffic rated manhole standard per UFC3-550-01 and associated UFGS sections.

c. Provide and install new 48” x 96” two door aluminum vault lids on each manhole. The lids shall be equipped with torsion assisted hinges, bolted down frames, recessed lock box, self-locking safety arms, and 6” x 6” x 3/8” reverse angle frame w/ anchor straps. The lids shall be coated w/ zinc rich epoxy powder.

1.2 SPECIAL REQUIREMENTS

· Coordinate disposal of all recyclable metal and other construction debris with the NBVC Recycling Center for proper disposal. The point of contact at the Recycling Center is Mr. Lloyd Nash at (805) 989-9899.

· All construction waste must be disposed properly in accordance with all federal, state, and local regulations and NBVC instructions.

· Please contact (805) 982-1970 or DSN: 551-1970 for hazardous waste disposal guidance and support.

· Maintain a clean job site and ensure that all required personal protective equipment is utilized throughout the execution of this project.

· Test and verify all new installations for proper operations.

· Site cleanup must be completed and inspected by KO or COR before move out.

· Access to the sites is not that difficult on foot. Incline is not bad, just takes time to access. Plants and bushes are not more than waist high. Paths and walkways are available in some spots thanks to Deer and other wildlife. There is no access via 4-wheel drive as the mountain area has a few culvert type, dirt troughs and small valleys that would cause a vehicle to become easily stuck. There is no way for a vehicle to enter this area, only hiking. Land is private property and no permission is granted to drive in this area. Total distance from bottom hand hole to top hand hole is ¼ to 1/2 mile. The bottom hand hole requires the use of a small rope (existing) to make about a 25-foot climb up a concrete culvert. Hand holes 4 & 5 are more difficult due to distance from top and bottom.

· Condition of the vaults: All vaults are in bad shape. We captured the worst cases with the pics, especially vault 6 (if annotated). All lids and metal access doors are all in about the same condition - not much difference there. Various concrete repairs will have to be made in the areas where door hinges and lids will be connected to the concrete vault. Current metal framing MIGHT have to be removed to make way for the new door system. Concrete will have to be strengthened in that area too.
