PERFORMANCE WORK STATEMENT

Naval Hospital Camp Pendleton

1. BACKGROUND: Requesting booths at 52 Area Brach Health Clinic (ABC)and Yuma Branch Health Clinic to conduct diagnostic audiological testing in addition to medical surveillance audiology testing. Requesting for 13 and 22 ABC larger booths to keep up with patient load demands.

These need to be set up to have DoD software and equipment to run through them. .The booths we order must have particular jack panels with 9 pin connector and 2" plug hole with snap cover this is included in a Benson jack panel in order to fun DoD microprocessor audiometer equipment. This is the requirement for medical surveillance.

For booths that included diagnostic must come with master jack on exterior of station and station jack on interior.

Additionally, proper window placement is needed in order to see patients.

The ones for 52 area branch clinic (ABC) must be primary diagnostic and secondary be able to test for medical surveillance. This must be double walled. Test up to four people.

Must include screening and diagnostic connection panel.

13 and 22 ABC must test up to six people in medical surveillance

Yuma must be double walled with primary medical surveillance and secondary diagnostic testing. For medical surveillance test four people. Must include screening and diagnostic connection panel.

4" minimum clearance is required between the exterior testing booth and the host facility wall and ceiling Vibration isolator rails need to be provided Ventilation 140 CFM system comprised of space saver exhaust intake air ceiling silencer panels and 120V, 60Hz, 14W intake fans

(2) 120V recessed lights LED lighting

(1) 120 V dimmer switch Carpet in all booths Perforated interior walls 120V electrical service Acoustically secure cable pass-through with finished appearance

The company must disassemble current one and dispose of old booth The company must assemble booth

To include divider curtains To include padded seats

All booths must be ANSI and DON requirements for each test frequency for medical surveillance and/or diagnostic testing ears covered and ears uncovered.

Door must have push plate and pull handle and magnetic door seals

Meet life safety measures.

Ability for sprinkler service for installation
