SUMMARY OF CHANGES
AMENDMENT 0003
The purpose of this Amendment is to:
1. Extend the response date from 20 November 2018 to 27 November 2018.
2. Amend Section L; PROPOSAL PREPARATION REQUIREMENT to remove any references inadvertently included regarding electronic submission of proposals. Please Note; Electronic Proposals will NOT be accepted.
All proposal must be mailed to the address specified in Block 9 of the RFP. Sealed offers in original and 2 copies for furnishing the supplies or services in the schedule will be received at the place specified in block 9 of the RFP, or if handcarried, in the depository located in Building 4 Second Floor until 04:00 PM local time on 27 November 2018.
3. Answer the following questions:
Q.) Within the Q&A its stated that the entire proposal (excluding price), resumes and letters of intent are excluded from the 25-page limit. Are Past Performance References provided ALSO excluded? Would these Past Performance References be counted within the 25 page?
A.) The 25-page limit referenced applies to the Technical Proposal. Past Performance is a separate factor.
Q.) Just to confirm. Can we can submit this electronically to you by e-mail?
A.) Regarding Proposal Submission; The Government inadvertently requested proposals be submitted both electronically and via hard copy in the original RFP. Sealed offers in original and 2 copies for furnishing the supplies or services in the schedule will be received at the place specified in block 9, or if handcarried, in the depository located in Building 4 Second Floor until 04:00 PM local time on 27 November 2018. Electronic proposals will NOT be accepted.
Q.) Do e-mails fulfill the requirement for acknowledgement of amendments?
A.) Please refer to Block 11 on each Amendment of Solicitation.
Q.) Our lab is NOT located inside CONUS. I assume this means we are not eligible for contract award? Is there any way to obtain an exception?
A.) Please refer to the response provided in Amendment 0002 and also refer to page 73 of the RFP, which states “The Offeror’s sampling laboratory shall be inside CONUS.”
SECTION A - SOLICITATION/CONTRACT FORM

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
PROPOSAL PREPARATION REQUIREMENT
It is requested that Offerors prepare their proposals in accordance with the following organization, content and format requirements to assist the government in making a complete and thorough evaluation of all proposals. Proposals shall be submitted as two separate documents, as follows:
Documents Original Copies Solicitation, Offer and Award Document 1 1 Technical Proposal 1 2 Cost Proposal 1 2
The “originals” shall be clearly identified as the “ORIGINAL”, and bear the original signature(s) of the Offeror. The “copies” shall be complete and clearly identified as “COPY” or “DUPLICATE”.
1.0 GENERAL
In order to facilitate the evaluation process, it is requested that Offerors also submit their cost proposal spreadsheets on compact disk (in addition to the hard copy requirements stated above). It is requested that spreadsheet files be compatible with Microsoft Windows 2000 Professional, Microsoft Office Excel 2003. The submission of spreadsheet files on compact disk in no way relinquishes the Offeror’s responsibility to provide hard copies of the cost proposal.
Offerors shall submit questions requesting clarification of solicitation requirements via email within seven
(7) calendar days after solicitation issuance, as time may not permit responses to questions received after that date to be prepared and issued prior to the receipt of proposals. For proposal purposes, the estimated date of the Contract award is approximately 180 days after solicitation closing.
2.0 SOLICITATION, OFFER AND AWARD DOCUMENTS
This document, which may be used as part of the contract award document, shall be fully executed and returned as a separate document from the technical and cost proposals. Special attention should be taken to accurately enter the prices required in Section B, complete all Representations and Certifications in Section K and ensure that an authorized person signs the offer in Block 30a of Page 1.
The document SHALL NOT be embellished with any cover or binding. If the Offeror makes any qualifications to any provisions in the RFP, all such qualifications shall be listed in a cover letter to the proposal. Qualifications may also be annotated on the Solicitation, Offer and Award document, if such annotation is necessary to clarify the qualifications.
2.1 Proposals must be submitted no later than 4:00 PM Eastern Standard Time on the closing date specified on page 1 block 8 of the RFP. Offerors must comply with the detailed instructions for the format and content of the proposal. Proposals that do not comply with the detailed instructions for the format and content of the proposal may be considered in-complete and may render the Offeror ineligible for award. The Government requires a minimum of 180 calendar days acceptance period.
2.2 Proposal format: In order to maximize efficiency and minimize the time for proposal evaluation, it is required that all Offerors submit their proposals in accordance with the format and content specified.

The Government will evaluate the Past Performance of the Prime offeror and subcontractors proposed to perform a major portion of the requirement. The past performance evaluation factor assesses the degree of confidence the Government has in an Offeror’s ability to supply products and services that meet users’ needs, based on demonstrated record of performance.
For evaluation of Past Performance, the Government may use information from any sources available to it including, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases. Interviews with Program Managers, Contracting Officers, CORs, and the Defense Contract Management Agency may be utilized. The Government may review other relevant past performance information from other sources or conduct questionnaires tailored to the circumstances of the acquisition. Offerors should provide relevant past performance information for similar or related work under contracts completed during the last three years.
Each Offeror has the opportunity to provide in its proposal any information regarding its past performance of contracts similar to the Government's requirement that it would like the Government to consider. Such information may be in the nature of additional information to that which the Government has readily available, or which is already included in PPIRS or other databases, or which the offeror considers essential to the Government's evaluation or explanatory information of substandard or poor performance and the corrective actions taken to prevent a recurrence. The Government reserves the right to verify statements and representations made in an offeror's proposal. The offeror may include Federal, State and Local Government and private sector contracts.

In the event the Navy cannot obtain adequate past performance information from PPIRS regarding a particular offeror, the Navy may review other relevant past performance information from sources other than those identified by the offeror. If the offeror intends to use subcontractors, the past performance of major subcontractors may also be evaluated.
To obtain information, the Government may contact the points of contact listed by offerors, however the Government is not required to contact the points of contact provided by offerors and may limit the number of references it decides to contact. The Government is also free to contact references other than those provided by the offeror to evaluate past performance of offerors. The Government may also obtain past performance information from other sources, such as, the SBA, Better Business Bureaus, etc.
3.2.6 – FACTOR 6 – PRICE
Price - For Firm-Fixed-Price (FFP) CLINs, the Offeror should include all proposed unit prices and extended amounts of CLINs in Section B, Schedule of Supplies/Services.
