About this file

This document is a Naval instruction (COMFLEACTSASEBOINST 11262.1L) issued by the Department of the Navy, Commander Fleet Activities Sasebo, Japan on 6 March 2020, establishing the management of Weight Handling Equipment (WHE) program at the facility and its departments and tenant commands. The instruction defines responsibilities and procedures for ensuring safety and reliability of all crane and rigging operations within COMFLEACT Sasebo and its outlying areas. It applies to Category 1, 2, 3 (cab and non-cab operated), and 4 cranes but excludes Naval Facilities Engineering Command Far East (NAVFACFE) Public Works Department and Ship Repair Facility (SRF) Sasebo, which operate under separate programs.

The instruction establishes compliance requirements with NAVFAC P-307, ASME B30 standards, OSHA regulations (29 CFR 1917, 1918, 1919.70), Japanese Industry Standards (JIS), and applicable Government of Japan standards. Key responsibilities are assigned to crane operators, work center supervisors, contracting officers, NAVFAC PWD Sasebo Transportation's Maintenance, Inspection, Testing and Certification (MITC) department, and the Public Works Officer (PWO). Contractors must provide advance notification (three working days for routine lifts, fifteen working days for critical lifts), submit Certificate of Compliance documentation, obtain contractor crane passes, and undergo initial inspections of equipment and rigging gear prior to operations. The instruction requires critical lift plans at least 15 days in advance for lifts exceeding 75 percent of crane capacity, multi-crane lifts, personnel lifts, lifts near power lines, crane erection, and other specialized operations. All crane and rigging accidents must be reported to the Navy Crane Center, with major incidents (fatalities, in-patient hospitalization, overturned cranes, collapsed booms) reported within 8 hours and other accidents within three working days. The instruction includes nine enclosures providing checklists, pass templates, accident report forms, and pre-use/daily operational inspection checklists for different crane categories.

( 4) CAT 4 Cranes. Includes commercial truck mounted cranes, articulating boom cranes, pedestal mounted conrn.1ercial boom assemblies (fixed length, telescoping and articulating types) attached to stake trucks, trailer, flatbeds, or railcars, or stationary mounted to piers with Original Equipment Manufacturer (OEM) rated capacities of 2,000 pounds or greater.

b. MPM. Equipment that are sometimes used as cranes to lift loads suspended by rigging gear. This equipment is sometimes referred to as construction equipment (rough terrain forklifts, telescopic handlers, bucket loaders, excavators, etc.) or material handling equipment (forklifts).

c. Load Bearing Components. These components of WHE that support the load and upon failure, could cause dropping, uncontrolled shifting or movement of the load.

d. Load Controlling Components. These components of WHE that position, restrain, or control movement of the load. A malfunction of these parts can cause dropping, uncontrolled shifting, or movement of the load.

e. Operational Safety Devices. Devices that affect the safe load lifting and handling capability of the equipment such as interlocks, limit switches, load/load moment and overload indicators with shutdown capability, emergency stop switches, radius indicating devices and locking devices.

f. Crane Rigging Gear and Miscellaneous Equipment. This is equipment that is used in weight handling operations. Rigging gear includes slings, chains, wire rope, metal mesh, synthetic rope, synthetic webbing and synthetic wound slings, links, shackles, eyebolts, swivel hoist rings, turnbuckles, portable load indicators such as dynamometers, load cells, crane scales, chain falls and hoists. Crane structures such as jib cranes, bridge cranes, monorails and davits that are not permanently mounted hoists are considered rigging gear. Crane rigging gear can include other devices such as spreader beams, plate clamps, magnets, vacuum lifters, container

COMFLEACTSASEBOINST 11262.1 L

spreaders, personnel platforms, portable A-frames, portable gantries, portable floor cranes.

Cranes and hoists that are procured, with, integral to and used solely in support of larger attachments integral to a piece of equipment such as threaded holes, welded lift lugs and original OEM (provided rigging gear is used for limited lifts such as offload, initial storage, reloading and shipment of that OEM product) are not considered rigging gear.

g. Crane Accident. A crane accident occurs when any of the elements in the operating envelope fails to perform correctly during operation, including operation during maintenance or testing resulting in: personnel injury or death, material or equipment damage, dropped load, derailment, two-blocking, overload, or collision (including unplanned contact between the load, crane and/or other objects).

h. Rigging Gear Accidents. A rigging gear accident occurs when any of the elements in the operating envelope fails to perform correctly during weight handling operations resulting in:

personnel injury or death, material or equipment damage, dropped load, two-blocking or overload.

i. Near Misses and Other Unplanned Occurrences. Near misses and other unplanned occurrences with lessons to be learned that do not fall under the crane and rigging gear accident definitions. A.near miss is a situation where an accident was avoided by mere chance or where intervention prevented an ongoing sequence of events that would have resulted in an accident.

j. Cargo Transfer. Per reference (a) the loading, unloading, moving or handling of cargo into, in, on, or out of any vessel. Cargo transfer operations performed with the use of shore based cranes and derricks are governed by OSHA 29 CFR 1917. Mobile cranes placed aboard vessels for cargo transfer will continue to be governed by OSHA 29 CFR 1918. Other lifting, such as repair parts, spare parts, etc. , associated with shipbuilding, ship repair or ship breaking, is not cargo transfer.

k. Lockout/Tagout. References (a) and G) covers the use of the Lockout/Tagout procedures.

Local instructions may be promulgated and followed but must not conflict with reference (g).

6. Responsibilities

a. General. All COMFLEACT Sasebo departments and tenant commands within the COMFLEACT Sasebo area of responsibility that utilize WH will adhere to the requirements of this instruction to include references (a), (d), (e), (g), (h) and (1).
