The dollar value is based on the amount for standalone projects or multiple projects that collectively equate to $200,000 or more per year and were on-going concurrently. Contracts listed may include those entered into with Federal, State, or local governmental agencies, as well as contracts with commercial entities. Submit a separate list for each subcontractor that will perform major or critical aspects of the requirement.
(2) If the evaluation of your Corporate Experience List (your firm's and your subcontractors') does not result in an Acceptable rating for Corporate Experience, the Government will consider the experience of your (your firm’s and your subcontractors’) Project Manager and Quality Manager. Submit Attachment L-2, “Key Personnel Experience List.” Include your Project Manager’s and Quality Manager’s relevant management experience for projects for maintenance and repair of fire protection and alarm systems (Fire Protection Systems, Automatic Sprinkler Systems, Foam Extinguishing Systems, FM-200 Fire Suppression Systems, Kitchen Fire Suppression Systems), service call work (emergency, urgent and routine service calls), and preventive maintenance for all fire protection and fire alarm system contracts and subcontracts), which were completed no more than seven years prior to the issuance date of this solicitation, or are currently in progress, and valued at $200,000 or more per year. The Project Manager and Quality Manager shall each have at least three years of relevant management experience. You must submit the “Key Personnel Experience List” for both the Project Manager and Quality Manager. If you submit the list for only one position, it will not be considered.
(2) Factor 2, Safety
(a) Solicitation Submittal Requirements: The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each contractor who is part of the partnership or joint venture; however, only one safety narrative is required. EMR, DART and TRC Rates shall not be submitted for subcontractors.)
(1) Experience Modification Rate (EMR): Include three previous complete calendar years’ worth of data (January 2015-December 2017). This rate compares company’s annual losses in insurance claims against its policy premiums. The offeror shall submit an insurance company-provided equivalent rate if no EMR exists. If the offeror has no EMR or equivalent rate for any year, the offeror must affirmatively state so and provide an explanation. Any extenuating circumstances that affect the EMR and negative trends must be addressed as part of the factor submission.
(2) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate and Total Recordable Case (TRC) Rate: Submit three previous complete calendar years’ worth of data (January 2015-December 2017). If an offeror cannot submit an OSHA DART and TRC rates, for any year, the offeror must affirmatively state so and provide an explanation. Any extenuating circumstances that affected the OSHA, DART, and TRC rate data negatively must be addressed as part of this factor submission.
(3) Technical Approach for Safety: Describe the plan for safety that will be implemented to evaluate safety performance of potential subcontractors as part of the selection process for all levels of subcontractors. The offeror shall submit a narrative that fully describes the safety management system that they will use to oversee the safety compliance and performance of self-performed and subcontractor performed work.
The offeror shall describe any innovative methods that will be employed to ensure and monitor safe work practices at self-performed and/or all subcontractor levels. Additionally, the contractor shall describe their methodology to execute an effective program that facilitates sound mishap prevention techniques and processes, employee reporting of unsafe conditions, unsafe activities, and near-miss mishaps. The technical approach to safety narrative shall be limited to two pages.
(3) Factor 3, Licenses and Certifications
Solicitation Submittal Requirements: The Offeror shall provide evidence of current NICET Level III licensing and certification(s) per Section C, 1502000, Spec Item 2.2.1, for each employee who will perform testing, inspection, and maintenance services.
(4) Factor 4, Past Performance
(a) Solicitation Submittal Requirements:
(1) Solicitation Submittal Requirements: IF A COMPLETED CPARS EVALUATION IS AVAILABLE, IT SHALL BE SUBMITTED WITH THE PROPOSAL. IF THERE IS NOT A COMPLETED CPARS EVALUATION, the Offeror shall submit the Past Performance Questionnaire (PPQ) included in the solicitation for each project the offeror includes in its proposal for Factor 1, Corporate Experience).
AN OFFEROR SHALL NOT SUBMIT A PPQ WHEN A COMPLETED CPARS IS AVAILABLE.
(2) IF A CPARS EVALUATION IS NOT AVAILABLE, the Offeror shall ensure correct phone numbers and email addresses are provided for the client point of contact. Completed PPQs should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror shall complete and submit with the proposal the first page of the PPQ (Attachment L-3), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government's point of contact, Ms. S. Sekiguchi, via email at sandy.sekiguchi@navy.mil prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs or CPARS previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.
(3) Offerors may also include performance recognition documents such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition if they were received within the last seven years prior to the issuance date of this solicitation.
(4) In addition to the above, the Government may review any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS) using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror’s proposal, inquiries of owner representative(s), and any other known sources not provided by the offeror.
(5) While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
d. Each offeror shall address and submit, as a minimum, ALL items required by the paragraphs above.
Failure to include a portion of any required item may render the proposal ineligible for award.
SECTION M - EVALUATION FACTORS FOR AWARD
The following have been modified:
EVALUATION CRITERIA
a. The major evaluation criteria are Price and Non-price.
b. The distinction between Corporate Experience and Past Performance is Corporate Experience pertains to the types of work and volume of work completed by a contractor that are comparable to the types of work covered by this requirement, in terms of value, scope, and complexity. Past Performance relates to how well a contractor has performed.
c. Price Proposal.
Basis of Evaluation: The Government will evaluate price based on the total price. Total price consists of the basic requirements and all option items (see Section B of the solicitation). The Government intends to evaluate all options and has included the provision FAR 52.217-5, Evaluation of Options (JUL 1990) in Section M of the solicitation. In accordance with FAR 52.217-5, Evaluation of Options will not obligate the Government to exercise the option(s). Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:
(1) Comparison of proposed prices received in response to the RFP.
(2) Comparison of proposed prices with the IGCE.
(3) Comparison of proposed prices with available historical information.
(4) Comparison of market survey results.
d. Non-price Proposal.
(1) Factor 1, Corporate Experience
Basis of Evaluation: The Government will evaluate the offeror’s relevant corporate experience on maintenance and repair of fire protection and alarm systems (Fire Protection Systems, Automatic Sprinkler Systems, Foam Extinguishing Systems, FM-200 Fire Suppression Systems, Kitchen Fire Suppression Systems), service call work (emergency, urgent and routine service calls), and preventive maintenance for all fire protection and fire alarm system contracts and subcontracts utilizing the “Corporate Experience List,” and Program Manager and Quality Manager relevant experience, to assess an offeror’s qualifications to manage and complete the requirements of the solicitation. The Government will consider Project Manager and Quality Manager experience (using the attachment entitled, “Key Personnel Experience List”) if its evaluation of the Corporate Experience List does not result in an Acceptable rating for Corporate Experience. The Government will consider Corporate Experience and Key Personnel Experience of subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the acquisition.
(a) A contract or subcontract will be considered relevant if
(1) it was similar in scope to the subject solicitation (i.e., maintenance and repair of fire protection and alarm systems (Fire Protection Systems, Automatic Sprinkler Systems, Foam Extinguishing Systems, FM-200 Fire Suppression Systems, Kitchen Fire Suppression Systems), service call work (emergency, urgent and routine service calls), and preventive maintenance for all fire protection and fire alarm system contracts and subcontracts), and
(2) it was completed no more than seven years prior to the issuance date of this solicitation, or is currently in progress, and
(3) it was valued at $200,000 or more per year. The dollar value is based on the amount for standalone projects or multiple projects that collectively equate to $200,000 or more per year and were on-going concurrently.
(b) The experience of your firm’s and your subcontractors’ firm’s Project Manager and Quality Manager will be considered relevant if
(1) The Project Manager and Quality Manager each have at least three years of relevant management experience for projects:
(i) Similar in scope to the subject solicitation (i.e., maintenance and repair of fire protection and alarm systems (Fire Protection Systems, Automatic Sprinkler Systems, Foam Extinguishing Systems, FM-200 Fire Suppression Systems, Kitchen Fire Suppression Systems), service call work (emergency, urgent and routine service calls), and preventive maintenance for all fire protection and fire alarm system contracts and subcontracts), and
(ii) Completed no more than seven years prior to the issuance date of this solicitation, or are currently in progress, and
(iii) Valued at $200,000 or more per year. The dollar value is based on the amount for standalone projects or multiple projects that collectively equate to $200,000 or more per year and were on-going concurrently.
(2) You submit the “Key Personnel Experience List” for both the Project Manager and Quality Manager. If you submit the list for only one position, it will not be considered.
(2) Factor 2, Safety
Basis of Evaluation: The evaluation will collectively consider the following:
- Experience Modification Rate (EMR)
- OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) and Total
Recordable Case (TRC) Rates
- Offeror Technical Approach to Safety
- Other sources of information available to the Government
(1) Experience Modification Rate (EMR): The Offeror shall submit EMR for the three previous complete calendar years (January 2015-December 2017). If no EMR exists, the offeror shall submit an insurance company-provided equivalent rate. If an insurance company-provided equivalent rate is provided, it will be treated the same as an EMR. If the offeror has no EMR or equivalent rate for any year, the offeror must affirmatively state so and provide an explanation. Any extenuating circumstances that affected the EMR and negative trends must be addressed as part of this selection factor. The board will evaluate the EMR to determine if the Offeror has demonstrated a history of safe work practices taking into account any negative trends and extenuating circumstances that impact the rating.
(b) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) and Total Recordable Case (TRC) Rates: The Offeror shall submit their OSHA DART and TRC rate, as defined by the U.S. Department of Labor, Occupational Safety and Health Administration, for the three previous complete calendar years (January 2015-December 2017). If the Offeror is unable to submit OSHA DART and TRC rates, the Offeror must affirmatively state so, and provide an explanation. Any extenuating circumstances that affected the OSHA DART and TRC rate data and negative trends must be addressed as part of this element. The board will evaluate the OSHA DART and TRC rates to determine if the Offeror has demonstrated a history of safe work practices taking into account any upward trends and extenuating circumstances that impact rates.
(c) Technical Approach to Safety: The Offeror shall describe their plan to evaluate their safety performance and that of potential subcontractors. The Offeror shall also describe any innovative methods that the Offeror will employ to ensure and monitor safe work practices at all subcontractor levels. The Safety Narrative shall be limited to two pages. The board will evaluate the narrative to determine the degree to which subcontractor safety performance will be considered in the selection of all levels of subcontractors on the upcoming project. The board will evaluate the narrative to determine the degree to which innovations are being proposed that may enhance safety on this procurement and validate the contractor has addressed methodology to be used in the execution of an effective program that facilitates sound mishap prevention techniques/processes, employee reporting of unsafe conditions, unsafe activities, and near-miss mishaps. Those Offerors whose plan demonstrates a commitment to hire subcontractors with a culture of safety and who propose innovative methods to enhance a safe working environment may be given greater weight in the evaluation.
(d) Other Sources of Information Available to the Government: Other sources for safety that the Government may review include, but are not limited to, OSHA data, NAVFAC’s Contractor Incident Reporting System (CIRS) in Enterprise Safety Applications Management System (ESAMS), Contractor Performance Assessment Reporting System (CPARS) and other related databases.
(3) Factor 3, Licenses and Certifications
Basis of Evaluation: The Offeror provided evidence of current NICET Level III licensing and certification(s) per Section C, 1502000, Spec Item 2.2.1, for each employee who will perform testing, inspection, and maintenance services.
(4) Factor 4, Past Performance
Basis of Evaluation: The degree to which past performance evaluations and all other past performance information reviewed by the Government (e.g., PPIRS, Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), performance recognition documents, and information obtained for any other source) reflect a trend of satisfactory performance considering:
- A pattern of successful completion of tasks;
- A pattern of deliverables that are timely and of good quality;
- A pattern of cooperativeness and teamwork with the Government at all levels (task managers, contracting officers, auditors, etc.);
- Tasks that are identical to, similar to, or related to the task at hand; and
- A respect for stewardship of Government funds

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT
YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating.
Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract.
There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(N) Not Applicable No information or did not apply to your contract Rating will be neither positive nor negative.
