Hello, All
The e‐mail below states that the Notice of General Permit Coverage HI R10F699 for FY18 MCON Project P‐704 was administratively extended. Though the e‐mail also indicates that additional processing steps may be required to complete processing of the online application, I didn't see anything on the online submission that indicated that there was any action required from us.
Attached is a copy of the online submission that shows that updated status to show under Active Steps: "NGPC Issued" and under "Status: "On Hold." According to the attached instruction from the State, if the status is "On‐Hold," then it means that the administrative extension was granted.
When the State issues the new NPDES general permit for storm water associated with construction activities, this project will be covered under that permit.
Very Respectfully, Anne
Anne Hong, P. E.

3. Authorized Representative Authorization
The Certifying Person hereby authorizes the named individual or any individual occupying the named position of the company/organization listed below to act as our representative to submit information/documents necessary to complete the NOI to discharge to State waters from the subject facility. Our representative is further authorized to submit information/documents for compliance with the NPDES general permit conditions, except submittal of the Notice of Cessation (NOC). The Owner hereby agrees to comply with and be responsible for all NPDES general permit conditions. This authorization begins with NOI processing and ends upon receipt of the NOC by the CWB. The Owner authorizes the duly authorized representative to submit additional information/documents necessary to complete the NOI and to submit information/documents to comply with the NPDES general permit conditions. The Owner is responsible for all information/documents submitted by the duly authorized representative for completion of the NOI and for compliance with the NPDES general permit conditions. The Certifying Person is required to sign the NOC for the project. After receipt of the NOC for the project, the duly authorized representative is no longer recognized by the CWB. The responsibility of the authorized representative cannot be delegated to an outside consultant with no financial responsibility for the company - they cannot sign as the "authorized representative" on behalf of the Owner. This requirement stems from the fact that self-reporting is critical under the Clean Water Act and Hawaii Water Pollution statutes; reports filed with CWB can have serious legal consequences, including possible civil and even criminal liability. The Owner in signing reports, therefore, must be represented by someone who has some responsibility for the corporation's financial interests. The Certifying Person attests that the authorized representative 1) meets the requirements of HAR 11-55-07(b); and 2) has financial responsibility within the corporation/organization who can attest to the accuracy of reports either because he or she participated in the preparation of the report, or supervises those who did prepare it and can attest that those individuals followed standard protocols that ensure the accuracy of the report. Both the Certifying Person and authorized representative understand that they can be subject to civil and criminal liability for non-compliance with NPDES general permit conditions, non-compliance with HAR Chapters 11-54 and 11-55, and for falsifying information.

Notice to NPDES Permittees of Hawaii Administrative Rules, Chapter 11-55 General Permits
Appendix C Storm Water Associated with Construction Activity Appendix L Circulation Water from Decorative Ponds or Tanks
This notice applies only to Hawaii Administrative Rules (HAR), Chapter 11-55, Appendix C, Storm Water Associated with Construction Activity and Appendix L, Circulation Water from Decorative Ponds or Tanks, both expiring December 5, 2018.
CONTINUING COVERAGE UNDER THE GENERAL PERMITS:
To continue General Permit (GP) coverage for the Appendices listed above, the following must be received by the Department of Health, Clean Water Branch (DOH-CWB) by November 7, 2018:
1. 2018 Renewal Notice of Intent Form through the DOH e-Permitting Portal. One (1) form per NPGC.
The DOH e-Permitting Portal is located at: https://eha-cloud.doh.hawaii.gov/epermit/.
2. Hard copy print out of the 2018 Renewal Notice of Intent Form.
3. Transmittal Requirements and Certification Statement (downloaded through completing the 2018
Renewal Notice of Intent Form) signed by the Certifying Person.
4. Nonrefundable $500 filing fee either through the DOH e-Permitting Portal (PayPal) or by check payable to the “State of Hawaii.“
The DOH-CWB will not process the 2018 Renewal Notice of Intent Form until Items 1 - 4 above are received.
If you are not issued a new NGPC prior to the expiration date (December 5, 2018), your current NGPC will be automatically administratively extended, unless otherwise informed, if Items 1 - 4 listed above are received by the DOH-CWB by November 7, 2018. Under an administrative extension, the terms and conditions of the expired GP will continue to be effective until a new NGPC can be issued under the readopted GP.
Please note that the DOH-CWB will not be sending letters confirming administrative extensions. To verify if your current NGPC was administratively extended, log in to the DOH e-Permitting Portal to view the Submission Details of the 2018 Renewal Notice of Intent Form you submitted, which can be found under the History link. A status of “On-Hold” for your 2018 Renewal NOI Form means the administrative extension was granted.
TERMINATING COVERAGE UNDER THE GENERAL PERMITS:
To terminate coverage under the GP, please submit a CWB Compliance Submittal Form for Individual NPDES Permits and NGPCs form through the DOH e-Permitting Portal with the Notice of Cessation portion of the form completed after your discharge ceases.
OBTAINING NEW COVERAGE UNDER THE GENERAL PERMITS:
If the new GPs are not adopted prior to December 5, 2018, there will be no GP to obtain coverage under and you will have the following options:
1. Apply for a NPDES Individual Permit at least 180 days prior to the commencement of construction activity (Appendix C) or the discharge to state waters (Appendix L).
2. If you wish to obtain coverage under either GP in the event that the new GPs cannot be issued prior to the expiration of the current GPs, perform the following:
A. complete the current CWB NOI Form with all required information via the DOH e-Permitting
Portal as soon as possible to obtain coverage under the current GPs.
B. After receiving a NGPC, submit a 2018 Renewal Notice of Intent Form (as explained above) to ensure GP coverage after December 5, 2018 by means of an administrative extension.

Q34. Please clarify the intent of specifying Type V cement for this project. ACI 318 “Building Code” indicates in Tables 4.2.1 & 4.3.1, that for moderate sulfate exposure “class S1” (seawater, Water soluble sulfate (SO4) in soil, percent by weight {0.10 < SO4 < 0.20} or Sulfate (SO4) in water {150 ppm < SO4 < 1500 ppm}) Type II cement is required. Unless the owners are expecting sulfate concentrations greater than “moderate” as defined by ACI 318, there is no need for Type V cement. If higher sulfate is expected, Hawaiian Cement can offer Type II cement with Class “F” fly ash to improve the sulfate resistance. Section R4.3 (Requirements for concrete mixtures) of ACI 318 states “The use of fly ash (ASTM C618, Class F), natural pozzolans (ASTM C618, Class N), silica fume (ASTM C1240), or ground-granulated blast-furnace slag (ASTM C989) also has been shown to improve the sulfate resistance of concrete.” The footnote on Table 4.3.1 states “‡For seawater exposure, other types of portland cements with tricalcium aluminate (C3A) contents up to 10 percent are permitted if the w /cm does not exceed 0.40.” Again, there is no requirement for type V. Remember that type V cement is for sulfate resistance only and does not improve resistance to chemical attack (acids) or corrosion (hydrogen sulfide in sewage treatment). If this is the intended purpose for the Type V cement, again, the addition of class “f’ fly ash can greatly improve chemical and corrosion resistance by decreasing the permeability of the concrete.
A.34. See answer to Question 7.

Hawaii has Sewer System maps which indicate manhole ID's, distance between manholes, top of manhole elevation, manhole invert elevation, and pipe size.
Q47. Why is Type V cement specified for this project and for this particular applications?
ACI 318 “Building Code” indicates in Tables 4.2.1 & 4.3.1, that for moderate sulfate exposure “class S1” (seawater, Water soluble sulfate (SO4) in soil, percent by weight {0.10 < SO4 < 0.20} or Sulfate (SO4) in water {150 ppm < SO4 < 1500 ppm}) Type II cement is required. Unless the owners are expecting sulfate concentrations greater than “moderate” as defined by ACI 318, there is no need for Type V cement. Please refer to the attached copy of ACI 318 Tables 4.2.1 & 4.3.1. If higher sulfate is expected, Hawaiian Cement can offer Type II cement with Class “F” fly ash to improve the sulfate resistance. Section R4.3 (Requirements for concrete mixtures) of ACI 318 states “The use of fly ash (ASTM C618, Class F), natural pozzolans (ASTM C618, Class N), silica fume (ASTM C1240), or ground-granulated blast-furnace slag (ASTM C989) also has been shown to improve the sulfate resistance of concrete.” The footnote on Table
4.3.1 states “‡For seawater exposure, other types of portland cements with tricalcium aluminate (C3A) contents up to 10 percent are permitted if the w /cm does not exceed 0.40.” Again, there is no requirement for type V.
