"I certify that on . . . . [insert date], the . . . . [insert Contractor's name] furnished the supplies or services called for by Contract No. . . . . via . . . . [Carrier] on . . . [identify the bill of lading or shipping document] in accordance with all applicable requirements. I further certify that the supplies or services are of the quality specified and conform in all respects with the contract requirements, including specifications, drawings, preservation, packaging, packing, marking requirements, and physical item identification (part number), and are in the quantity shown on this or on the attached acceptance document."

Date of Execution:

Title:

Q - Is this referring to the certification that the DCMA QAR "signs" before the engine can ship? If so, the Carrier and bill of lading or shipping document have not been generated yet because they can't until the DCMA QAR has completed their inspection and acceptance.

If it is not referring to the Gov't Source Inspection certification, then what is the intent?

A - No, this is a separate document from Gov't Source Inspection Certification. The Certificate of Conformance is a FAA regulation and required for all FAA Certified Repair Stations. The intent is to be in compliance with regulations, and for the contractor certify that the product or service has been completed in accordance with contract requirements.

2. CDRL A004 (Program Office Certification Worksheets (POCW)); SOW para 6.2.1

Q - It is not clear what this CDRL is referring to. Can an example of the Program Office Certification Worksheet be provided to assist with knowing what the requirements are?

A – This CDRL is to document any change in engine/module reporting status. Please see attached sample POCW.

3. CDRL A006 (Retrograde Report)

Q - Was a Sample T700 Retrograde Report.xlsx provided as an attachment? If so, which one? If not, can it be?

A – The sample was inadvertently not included. Please see attached Sample T700 Retrograde Report.
