AMENDMENT 0004

The purpose of this amendment is to:

1. Extend proposal due date

2. Incorporate a revised Attachment H Price Proposal Form

3. Incorporate PPI responses

4. Incorporate Drawing revisions

5. Incorporate Specification revisions

1. The proposal due date is extended from 08 June 2023 at 2:00PM EDT to 13 June 2023 2:00PM EDT

2. Incorporate revised Attachment H Price Proposal Form

• Attachment H Price Proposal Form Revised is attached separately

3. Incorporate the following PPI Responses

Date submitted Question Gov’t Response

06/07/23 The descriptions for what is to be included in each of these line items (0001E and 0001F) is very close to the same description leading us to believe that they may be addressing the same scope of work. Also, on the bid form for item 001F there is no space to include: quantity, unit, unit price, and total price blank to fill in on the form. Is the description included for 0001F simply further describing the cost to be included in the cost line for 0001E? - if this is the case we will just include the cost for the complete scope 0001F in the 001E line item.
