AMENDMENT 0004
Proposal Inquiry Questions and Answers
1. J-0200000-14 Block 4 must be checked to show if the project is at least 80% complete or not. It’s not clear what relevance this has to the evaluation. Why is 80% significant?
Answer: Please see Revised Attachment J-0200000-14
2. Att J-0200000-14
Block 10 must show the evaluation received. Why is this required, since CPARS or PPQs are required? This would give us more space for the project narrative.
Answer: Please refer to the requirements of the RFP. Additional information will be obtained from the CPAR/PPQs per Factor 4- Past Performance on Recent, Relevant Projects
3. RFP Section L
RFP pages 260 and 266 lists different sets of requirements for Price Proposal. Which list of items should we use?
Answer: See Amendment 0003
4. RFP Section L
RFP pages 260 and 266 lists the price volume as the 1st and 3rd things in the list. Is
Price Proposal Volume 1 or Volume 3?
5. RFP Section L
All Factor paragraphs in RFP L.4 have a Basis of Evaluation. Sometimes, the Section L
Basis of Evaluation is different than the Section M evaluation criteria. Please clarify how each factor will be evaluated.
6. RFP Section M
All Factor paragraphs in RFP M.1.B have Submittal Requirements. Often, the Section M
Submittal Requirements is different than the Section L requirements. Please clarify what must be submitted for each factor.
7. Att J-0200000-13 ELIN Sheet Recurring Work x001AD indicates “FSRM Routine Service Orders 3.1.5.” FSRM is not used in PWS
3.1.5 and not found in the definitions and acronyms lists. What is FSRM and how does it apply to 3.1.5?
Answer: These service orders are bound to the information provided in 1502000C spec item 3.1.5 with the same limiit of liability, and response and completion times unless otherwise defined in the related information.
8. Att J-0200000-13 ELIN Sheet Recurring Work x001AE indicates “BSS1 Routine Service Orders 3.1.5.” BSS1 is not used in PWS 3.1.5 and not found in the definitions and acronyms lists. What is BSS1 and how does it apply to 3.1.5?
Answer: These service orders are bound to the information provided in 1502000C spec item 3.1.5 with the same limiit of liability and response and completion times unless otherwise defined in the related information.
9. Att J-0200000-13 ELIN Sheet Recurring Work
Verify the correct spec item for A001AL is 3.2.3.1.2
Answer: Elin sheets have been updated.
10. Att J-0200000-13 ELIN Sheet Recurring Work x001AM, x003AA, and x004AC all indicate “Vertical Transportation Equipment (VTE)
3.2.4 and 3.3.8 (15).” This appears to be a combined PM and ITC requirement in the same ELIN. Request the Government split each of these into two separate ELINs like the other PM and ITC items.
Answer: Combine cost for total VTE work.
11. Att J-0200000-13 ELIN Sheet Recurring Work
ELIN x001AG is duplicated on rows 19 and 21 of the ELIN sheets. Please renumber to eliminate the duplicate ELIN.
Answer: Elin sheets have been updated.
12. Att J-0200000-13 ELIN Sheet Recurring Work
ELINs x001AQ, AU, AX, AZ, BA, BB, x003AB, and x006AA all list the unit of issue as “A” however “A” is not included on the NAVFAC UOM tab. Please verify that “A” indicates annual.
Answer: A represents Annual- Refer to the Frequency of Services- Section J- 0200000-01- Definitions and Acronyms
13. Att J-0200000-13 ELIN Sheet Recurring Work
ELINs x001AS and x001BC list the unit of issue as “Q” however “Q” is not included on the NAVFAC UOM tab. Please verify that “Q” indicates quarterly.
Answer: Q represents Quarterly- Refer to the Frequency of Services- Section J- 0200000-01- Definitions and Acronyms
14. Att J-0200000-13 ELIN Sheet Recurring Work
The ELIN description for x001AT says “Base year only” however this ELIN is listed in each year’s recurring work. Please clarify if this is a requirement beyond base year.
Answer: After Elin sheet update the number is 001AU. This requirement is only for the base year.
15. Att J-0200000-13 ELIN Sheet Recurring Work
ELIN quantities in Column C for x001BD-x001BE do not match the description in Column B. Please resolve the conflicts between description and quantity.
Answer: The quantities for the base year is correct. Bid accordingly.
16. Att J-0200000-13 ELIN Sheet Recurring Work
ELINs x001BL and x001BM show units of issue as “EA” for removal of refrigerant;
however, no other information is provided in the solicitation as to the quantity of refrigerant removed each time it is ordered. In order to properly estimate and price these ELINs, please provide the quantity in pounds of refrigerant to be removed, either on average per removal, or total to be removed per year.
Answer: Small systems are refrigerators, window ac units, and drinking fountains.
Typically smalle systems contain five pounds or less of refrigerant. Systems with more than five pounds are considered large systems for this requirement.
17. Att J-0200000-13 ELIN Sheet Recurring Work
The description and quantity listed for ELIN x723 are insufficient to define a scope of work for pricing purposes. It is unclear if the quantity of six (6) means the three ponds listed in J-1502000-31 are serviced 2 times each under this ELIN or if these are additional ponds, or specific ponds. Please clarify the aquatic weed control requirement.
Answer: Elin has been updated to better define the requirement.
18. Att J-0200000-14 All
What is the font size requirement for filling out this two-page Government form?
Answer: Please refer to Amendment 0003
19. Att J-0200000-14 4. Award Date
The form asks if the project is at least 80% complete; however, the evaluation criteria is silent on how this impacts the project
Answer: Refer to Factor 2- Recent/Relevant Experience pg 272- 3rd para from the bottom. There is no impact as the RFP states "shall be ongoing".
20. Att J-0200000-14 10. Evaluation Received The form requires the offeror to enter the evaluation received for the project in Factor 1.
Since RFP L.4(4) requires CPARS, PPQs and recognition documents under Factor 4, request the Government delete block 10 on Attachment 0200000-14 since the Factor 1 response is page limited.
Answer: See Revised Attachment J-0200000-14
21. Att J-0200000-14 15. Other Information
Please clarify if the form is either missing item number 14 or block 15 is incorrectly numbered.
Answer: See Revised Attachment J-0200000-14
22. RFP Section B, pp. 4-5 B.18 Contractor’s Payment Schedule
Contractor’s Payment Schedule submission requirements are not listed in Section L as indicated in B.18. Please provide instructions on submission of Contractor’s Payment Schedule.
Answer: Please refer to Amendment 0003- Section B- B.18
23. RFP Section B, pp. 7-10 Schedule of Supplies and Services
The Government lists “Each” on the Section B schedule and within the individual ELINs. This complicated methodology creates significant manpower inefficiencies for the Government and the Contractor to track each event to an invoice. This also makes contract costs unpredictable month-to-month. Would the Government consider a firm, fixed price for each period of performance divided into equal monthly payments (Quantity of 12, Unit of Month) for each
CLIN?
Answer: This procurement is performance based. As a result, monthly payments will vary, according to the work performed.
24. RFP Section C, p. 58 1502000, Spec 3.0 Service Orders
RFP states; “Service orders will not be issued for accomplishment of repairs on systems and equipment maintained under IMP.” Please verify that no assets on this contract are subject to an integrated maintenance program.
Answer: Currently no equipment is under IMP
25. RFP Section C, p. 66 1502000, Spec 3.2 Preventive Maintenance
The limit of liability in 3.2 is described as $250 in direct labor and materials. The limit of liability in 3.1 service orders is defined as total labor and materials. Why are the limits of liability calculated differently for PM and Service Orders?
Answer: The $250.00 limit of liability as described in 3.2 involves extra work performed by a tradesman if the individual finds an issue while performing a service (PM). As for the service order work, there are many other participants involved and possibly more than one trade needed to complete the work.
26. RFP Section C, p. 67 1502000, Spec 3.2.2 Boilers and UPVs
Spec item states; “The boilers and UPVs inventory and certification dates are provided in J-1502000-06.” However, it appears the PWS does not require certification services and there are no certification dates on J-1502000-06. Please confirm whether the contractor is or is not responsible for boiler and UPV certification.
Answer: Please refer to Annex 1502000 C spec item 3.2.2 Performance Objective.
27. RFP Section C, p. 75 - 1502000, Spec 3.2.8 Paint Stripping Booth Equipment Spec item states; “The Contractor shall be responsible for maintaining the inventory of paint stripper material supplies.” Does this mean the contractor is responsible for the paint stripper chemicals? If so, please provide type materials and historical consumption for each location.
Answer: Please refer to 1502000C spec item 3.2.8. The amount of stripper used information is unavailable. In regard to the type of material, please refer to the type of equipment listed in the J section and utilize the appropriate chemicals.
28. RFP Section C, p. 77 1502000, Spec 3.2.11 Compressed Air Systems Atlas Copco
Spec item states; “The contractor shall provide all repair parts required to perform maintenance/repairs in accordance with the requirements of this PWS.” Additionally, it states; “Service Order response for air compressors only is as follows: The contractor shall respond to Emergency Orders within 8 hours and 24 hours for Routine and Urgent Service Orders.” Why are these service order requirements in the PM section? Is the limit of liability $2,500, $5000 or $250?
Answer: The limit of liability remains the same. The time for response for this spec item alone is different than that defined as Emergency, Urgent, and Routine do to the nature of this equipment.
29. RFP Section C, p. 79 3.3 Inspection, Testing, and Certification The related information and performance standard both mention PMs but this section is
Inspection, Testing, and Certification (ITC). The only ITC asset that appears to be also subject to PM is vertical transportation equipment. Please clarify how PMs are relevant to ITC and the ITC limit of liability.
Answer: While in the course of performing Inspection, Testing, and Certification, the Contractor may perform tasks associated with preventative maintenance. Due to the type of equipment listed, Inspection and testing are required to ensure the PM was performed properly. As for the limit of liability, refer to spec. item 3.3.
30. RFP Section C, p. 84 3.3.7 Calibration, and Certification Gas Monitoring
System IWTP B-4380 Related information in spec item 3.3.7 requires quarterly maintenance which appears to be a PM requirement but this section is Inspection, Test and Certification (ITC). If the Government intends for these two services to be combined, is there a separate $250 limit of liability for PM and ITC or does the one $250 ITC limit of liability apply?
Answer: Due to the type of equipment listed, Inspection and testing are required to ensure the PM was performed properly. As for the limit of liability, refer to spec.
item 3.3.
31. RFP Section E, p. 103 E.3 Government QA E.3 references a clause titled “Consequences of Contractor’s Failure to Perform Required
Services,” a clause that appears to have been omitted from the RFP. Please provide the clause or otherwise clarify the consequences.
Answer: Please see Amendment 0004- Revised Section E
32. RFP Section J, p. 208 J-1502000-04 J-1502000-04 lists “STRU?” Why does this have a question mark and what does the acronym mean?
Answer: The ? is a typo. STRU means structual.
33. RFP Section J, p. 229 J-1502000-18 ESS ADU Paragraph 4.2.2 of this attachment discusses unscheduled maintenance requirements.
Why are these service order requirements in the PM section? Is the limit of liability $2,500, $5000 or $250?
Answer: Service Order requirements are associated with all items under PM. In reference to the limits of liability and how they are associated, please refer to spec.
items 3.1, 3.1.2, and 3.2
34. RFP Section J, p. 240 J-1502000-29 Portable Toilet Service Attachment J-1502000-29 and spec item 3.4.4 do not give sufficient information to price
ELIN x001BJ. The frequency of “weekly” indicates the recommended servicing interval but the duration of time that each toilet will be in place is unclear. Are the portable toilets listed in Attachment J-1502000-29 continuously in place in the same location throughout the period of performance?
Answer: This is a Recurring Work (FFP) effort; please refer to 3.4.4 and J-1502000-
35. RFP Section L, pp.260 & 266 L.3 and L.4 Price Proposal
RFP paragraph L.3 (p. 260) lists one set of requirements for Price Proposal and L.4 (p.
266) lists a different set of requirements for Price Proposal. Please clarify the submission requirements and sequence of items for the Price Proposal.
Answer: Please refer to Amendment 0003
36. RFP Section L, pp.260 & 266 L.3 and L.4 Volumes RFP paragraph L.4(a) requires submission of separate volumes for price/cost and non-cost/price. Since L.3 (p. 260) and L.4 (p. 266) both show Price proposal requirements the sequence is unclear. Please clarify the volume numbers and volume titles for proposal submission.
Answer: Please refer to Amendment 0003
37. RFP Section L, p. 261 L.4(b)(1) RFP paragraph L.4(b)(1) specifies 12-point Arial or Times New Roman font. Request the
Government consider a provision to allow 10-point font for tables, graphics, headers, and footers for better readability and organization of offeror proposals.
Answer: Please refer to the requirements of the RFP- Section M- Non-cost/price Submission Requirements as stated in Amendment 0003
38. RFP Section L, p. 261 L.4(b)(1) RFP paragraph L.4(b)(1) specifies a 100-page limit and sequential page numbering but does not list any items exempt from page-count. Tab dividers cannot be readily numbered. Are tab dividers exempt from page count
Answer: Yes. Tab dividers are exempt from the page count
39. RFP Section L, p. 261 L.4(b)(1)(i) RFP paragraph L.4(1)(i) on p. 261 under Factor 1 states; “Each of the topics below must be included in the narrative and tabbed in the technical proposal and discussed separately.” Did the Government intend for each Factor in the Non-Cost/Price Volume to have a separate tab, or was it intended that each of the Factor 1 subparagraphs a.-f receive a separate tab?
Answer: Yes. Each Factor must have a separate tab. Factor 1 subparagraphs a-f should also be tab separately.
40. RFP Section L, p. 261 L.4(b)(1)b.
Please clarify the first sentence as it relates to the 10-page limit. Does the 10-page limit apply only to the workforce management plan portion of the response or does it apply to the entirety of L.4(1)b?
41. RFP Section L; p. 261 L.4(b)(1)b.
The last sentence in this paragraph is unclear. The subject of this section is key personnel; however, the requirements do not include Environmental Energy Manager (a key personnel position) and they do include on-site supervisors who are not key personnel. Please clarify the response requirements for L.4(1)b.
Answer: Please refer to revised Factor 1- Management Approach
42. RFP Section L, p.261 L.4(b)(1)f.
Please clarify the 2-page limit. Does the 2-page limit apply only to the Quality
Management System summary portion of the response or does it apply to the entirety of L.4(1)f.?
Answer: Please refer to Amendment 0003
43. RFP Section L, p.262 L.4(b)(2)i.
Submission requirements for Factor 2 in paragraph L.4(b)(2)i. appear to call for projects performed as prime contractor only; however, block 13 of Attachment J-0200000-14 appears to allow experience as a subcontractor. If experience is required as a prime contractor, could the Government delete block 13?
Answer: See Revised Attachment J-0200000-14.
44. RFP Section L, p. 266 L.4(1) RFP paragraph L.4(1) on p. 266 indicates that offers are solicited on “all or none” basis and FAR 52.215-1 has been modified; however, FAR 52.215-1(f)(1), (f)(6), and (f)(9) on
p. 271 appear to be in conflict with other solicitation requirement for single award firm, fixed price contract. Request the Government delete FAR 52.215-1 from the solicitation.
Answer: Please refer to Amendment 0003
45. RFP Section L, p. 262 L.4(2)(i) RFP paragraph L.4(2)(i) on p. 262 requires a two-page limit for Recent/Relevant
Experience. Within these page constraints, offerors can only demonstrate the ability to provide maintenance on the 26 items listed under “Scope” in a one-line sentence or an “x” on a chart. Request the Government increase the page count to four (4) pages for each project.
Answer: Please refer to the requirements of the RFP- Section M- Factor 2 Recent/Relevant Experience as stated in Amendment 0004
46. RFP Section L, pp.260 & 266 L.4 Basis of Evaluation Multiple paragraphs in RFP paragraph L.4 (p. 260-266) list a Basis of Evaluation section that in some cases differs from Section M evaluation criteria. Recommend moving Basis of Evaluation items to Section M and adjudicate any differences.
Answer: Please refer to Amendment 0003
47. RFP Section M, p. 274 M.1.A Basis for Award Several of the standards listed under FAR 52.215-1 (pp. 268-271) appear to conflict with the M.1.A Basis for Award standards listed on pp. 274-275. Request the Government delete FAR 52.215-1 from the solicitation.
Answer: Please refer to Amendment 0003
48. RFP Section M, pp.275-281 M.1.B Evaluation Factors for Award Multiple paragraphs in RFP paragraph M.1.B (p. 275-281) list a Submittal Requirements section that in some cases differs from Section L submittal requirements. Recommend moving Submittal Requirements items to Section L and adjudicate any differences.
Answer: Please refer to Amendment 0003

76. Requirements: Offerors shall submit the following information:, Scope: Demonstrate the ability to provide maintenance and repair services to a wide variety of systems and equipment. Offerors are required to demonstrate experience with Sewage Plant Sludge Removal and Land Application, Pond Aquatic Weed and Fisheries Management as defined by the RFP. Offerors may be considered more favorably by also demonstrating the following services: Herbicide application, Emergency, Urgent, and routine trouble call response, Sub Surface Utility Locating and Marking, HVAC and Boiler Water Testing and Treatment Services, maintenance of the following: controls for Boilers and Unfired Pressure Vessels (UPVs), CO2 Fire Suppression Systems and Gaylord/Ansul Hoods and Ducts, Vertical Transportation Equipment (VTE), Medical Gas components and associated equipment, AV8B Engine Test Stands, Biological Nutrient Removal (BNR) Actuator Valves, Paint Stripping Booth Equipment, Reverse Osmosis Systems, Ultra-Violet System, Hangar Floors, Compressed Air Systems, Grease Traps, Port-A-John Service, Sealed Vault Toilet/Septic Tanks. Offerors demonstrating a greater number of these services may be considered more favorably.
Question: Is the requirement to demonstrate experience with regards to “Sewage Plant Sludge Removal and Land Application, Pond Aquatic Weed and Fisheries Management” something that can be provided by the offeror in their proposal as a subcontracted service? Currently, we believe this service is being provided by a subcontractor in support of the prime contractor on this contract. We have been provided quotes from the existing incumbent subcontractors that are providing these services and will include them as part of our proposal and follow on service, if awarded the contract. The current requirement by the government serves to limit competition and provide an unfair advantage to the incumbent contractor, if they are allowed to utilize the past performance of the current subcontractor. We believe this needs clarification in order to allow offerors a fair and equitable opportunity to propose offers to the government. We request the government provide additional clarification on this requirement.
Answer: Refer to revised RFP- Section M- Factor 2 Recent/Relevant Experience- Amendment 0004
SECTION E - INSPECTION AND ACCEPTANCE
The following have been added by full text:
E.5
CONSEQUENCES OF CONTRACTOR'S FAILURE TO PERFORM REQUIRED SERVICES (OCT 2004)
The Contractor shall perform all of the contract requirements. The Government will inspect and assess Contractor performance in accordance with FAR 52.246-4, INSPECTION OF SERVICES – FIXED PRICE and the Section E provision entitled GOVERNMENT PERFORMANCE ASSESSMENT. The Government will require performance, withhold payment, or seek other suitable consideration for unsatisfactory or non-performed work.
When defects can’t be corrected by re-performance, the Government may reduce the price to reflect the reduced value of services performed.
(a) PROCEDURES. In the case of unsatisfactory or nonperformed work, the Government:
(1) may give the Contractor written notice of observed deficiencies prior to withholding payment for unsatisfactory or non-performed work and/or assessing liquidated damages. Such written notice shall not be a prerequisite for withholding payment for nonperformed work. The Government may specify, as provided for below, that liquidated damages can be assessed against the Contractor. Such liquidated damages are to compensate the Government for administrative costs and other expenses resulting from the unsatisfactory or nonperformed work.
(2) may, at its option, allow the Contractor an opportunity to re-perform the unsatisfactory or nonperformed work, at no additional cost to the Government. In the case of daily work, corrective action must be completed within 2 hours of notice to the Contractor. In the case of other work, corrective action must be completed within 24 hours of notice. In addition, the Government can assess liquidated damages, as referenced above, in the amount of 15 percent of the value of all observed defects. The original inspection results of the Contractor's work will not be modified upon re-inspection. However, the Contractor will be paid for satisfactorily re-performed work.
(3) shall withhold from the Contractor’s invoice all amounts associated with the unsatisfactory or nonperformed work at the prices set out in the Schedule or provided by other provisions of this contract, unless the Contractor is required to re-perform and satisfactorily complete the work. In addition, the Government can assess liquidated damages, as referenced above, in the amount of 15 percent of the value of all observed defects.
(4) may, at its option, perform the work by Government personnel or by other means. The Government will reduce the amount of payment to the Contractor, by the amount paid to any Government personnel (based on wages, retirement and fringe benefits) plus material, or by the actual costs incurred to accomplish the work by other means.
If the actual costs cannot be readily determined, the prices set out in the Schedule will be used as the basis for the deduction. In addition, the Government can assess liquidated damages, as referenced above, in the amount of 15 percent of the computed cost.
(b) The Contractor is responsible for maintaining an effective Quality Control Program during the course of the contract. Failure to maintain adequate quality control may result in Termination for Default.
(c) Re-performance by the Contractor does not waive the Government's right to terminate for nonperformance in accordance with FAR clause 52.249-8, “DEFAULT (FIXED-PRICE SUPPLY AND SERVICE)” of Section I and all other remedies for default as may be provided by law.
SECTION M - EVALUATION FACTORS FOR AWARD
The following have been modified:
M.1 BASIS FOR AWARD
EVALUATION FACTORS FOR AWARD
A. BASIS FOR AWARD
1. The Government reserves the right to eliminate from consideration for award any or all offers at any time prior to award of the contract(s); to negotiate with offerors in the competitive range; and to award the contract to the offeror submitting the proposal determined to represent the best value—the proposal most advantageous to the Government, price and other factors considered.
2. As stated in the solicitation, the Government intends to evaluate proposals and award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a)). The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. In addition, if the Contracting Officer determines that the number of proposals that would otherwise be in the competitive range exceeds the number at which an efficient competition can be conducted, the Contracting Officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals.
3. The tradeoff process is selected as appropriate for this acquisition. The Government considers it to be in its best interest to allow consideration of award to other than the lowest priced offeror or other than the highest technically rated offeror.
4. As stated in the solicitation, all technical factors when combined are of equal importance to the performance confidence assessment (past performance) rating; and all technical factors and the performance confidence assessment (past performance) rating, when combined are approximately equal to price.
5. Any proposal found to have a deficiency in meeting the stated solicitation requirements or performance objectives will be considered ineligible for award, unless the deficiency is corrected through discussions. Proposals may be found to have either a significant weakness or multiple weaknesses that impact either the individual factor rating or the overall rating for the proposal. The evaluation report must document the evaluation board’s assessment of the identified weakness(s) and the associated risk to successful contract performance resulting from the weakness(s). This assessment must provide the rationale for proceeding to award without discussions.
B. EVALUATION FACTORS FOR AWARD
1. The solicitation requires the evaluation of price and the following non-cost/price factors:
Factor 1– Management Approach Factor 2 – Recent, Relevant Experience of the Firm Factor 3 – Safety Factor 4 – Past Performance on Recent, Relevant Projects
NOTE: The distinction between Recent / Relevant Experience and Past Performance is that experience pertains to the volume of work completed by a contractor that are comparable to the types of work described under the definition of recent, relevant projects, in terms of size, scope, and complexity. Past Performance pertains to both the relevance of recent efforts and how well a contractor has performed on the contracts.
2. The relative importance of non-cost/price factors is that Factors 1, 2 and 3, are of equal importance to each other and, when combined, are equal in importance to the past performance evaluation/performance confidence assessment Factor 4. When the proposal is evaluated as a whole, the technical factors and past performance/performance confidence assessment factor combined (i.e., the non-cost/price evaluation factors) are approximately equal to price.
The importance of price will increase if the Offeror’s non-cost/price proposals are considered essentially equal in terms of overall quality, or if price is so high as to significantly diminish the value of a non-cost/price proposal’s superiority to the Government. Award will be made to the responsible Offeror whose offer conforms to the solicitation and represents the best value to the Government, price and non-price factors considered.
3. Basis of Evaluation and Submittal Requirements for Each Factor.
(a) Price:
(1) Solicitation Submittal Requirements: Offerors shall submit one (1) original and one
(1) copy of the price proposal, in a separate three ring binder, that shall include the following:
• Signed Solicitation, Offer and Award Forms (SF 33), including Section B (CLINs 0001 through 0010), with all amendments acknowledged.
• Exhibits A through E (Attachment J-0200000-13)
• Copy of Representations and Certifications and proof of registration in System of
Award Management (SAM).
• Confirm submission of the most recent Vets 4212 Report.
• Provide name of financial institution, point of contract, telephone number and email address. Provide a signed statement of release giving the reference permission to release the information to the Navy.
• In addition to providing a hard copy of the price proposals, offerors shall also provide one (1) non-rewritable CD with pricing information. Offerors are advised that in the event of a discrepancy between pricing information contained on the CD and the hard copy, the hard copy will govern.
The price proposal shall be submitted separately but at the same time as the technical proposal.
(2) Basis of Evaluation: The Government will evaluate price based on the total price.
Total price consists of the basic requirements and all option items (see Section B of the solicitation). The Government intends to evaluate all options and has included the provision FAR 52.217-5, Evaluation of Options (JUL 1990) in Section M of the solicitation. In accordance with FAR 52.217-5, evaluation of options will not obligate the Government to exercise the option(s). Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:
(i) Comparison of proposed prices received in response to the RFP.
(ii) Comparison of proposed prices with the IGCE.
(iii) Comparison of proposed prices with available historical information.
(iv) Comparison of market survey results.
(1) Factor 1 - Management Approach:
(i) Solicitation Submittal Requirements: The offeror shall submit a narrative response that clearly demonstrates its understanding of and approach to accomplishing the complexity and magnitude of service requirements set forth in the performance objectives and standards of the Performance Work Statement. c. Describe the work to be performed as the prime contractor as well as the work to be performed by each subcontractor proposed. The work to be self-performed, subcontractors to be utilized and their roles shall be clearly identified. If the prime will self-perform 100% of the work, it shall be clearly stated. Offeror must meet the requirements of DFARS 52.219-14, Limitation on Subcontracting. Submit Attachment J-0200000-16 as part of this factor.
d. Demonstrate your ability to provide and maintain a qualified workforce and clearly define the controls and/or processes that are in place to ensure continuity of services.
e. Demonstrate your ability to manage numerous work requirements simultaneously with a dispersed workforce under competing priorities at multiple locations.
f. Quality Management. Provide a summary of your Quality Management
System, not to exceed two (2) pages, that describes Quality Processes (Practices, Resources and Activities) and minimum controls that will be used to ensure full compliance with all performance objectives and standards as described in (0200000 - Management and Administration). Include the methodology to be implemented to accomplish the performance requirements and detail how the proposed approach will effectively achieve each performance objectives and standards.
(ii) Basis of Evaluation: The Management Approach factor shall be evaluated based upon the following criteria:
a. The proposal demonstrates an acceptable understanding of the performance objectives and standards.

Submit a minimum of one (1) and a maximum of five (5) relevant projects, where the offeror served (as the prime contractor) that are similar in size, scope, and complexity to the work requirements specified in the solicitation. For projects submitted, experience of proposed subcontractors will not be considered. Likewise, the Offeror’s experience as a subcontrator will not be considered.
Size, scope, and complexity are further defined as follows:
Size: A maintenance service contract with a yearly value of at least $1.8 million or greater per year for recurring services. Individual Indefinite Quantity / Indefinite Delivery task orders for non-recurring work will not be considered similar in size.
Scope: Demonstrate the ability to provide maintenance and repair services to a wide variety of systems and equipment to include: Sewage Plant Sludge Removal and Land Application, Pond Aquatic Weed and Fisheries Management , Herbicide application, Emergency, Urgent, and routine trouble call response, Sub Surface Utility Locating and Marking, HVAC and Boiler Water Testing and Treatment Services, maintenance of the following: controls for Boilers and Unfired Pressure Vessels (UPVs), CO2 Fire Suppression Systems and Gaylord/Ansul Hoods and Ducts, Vertical Transportation Equipment (VTE), Medical Gas components and associated equipment, AV8B Engine Test Stands, Biological Nutrient Removal (BNR) Actuator Valves, Paint Stripping Booth Equipment, Reverse Osmosis Systems, Ultra-Violet System, Hangar Floors, Compressed Air Systems, Grease Traps, Port-A-John Service, Sealed Vault Toilet/Septic Tanks as defined by the RFP.
