CHECKLIST FOR NAVAL FACILITIES ENGINEERING COMMAND WASHINGTON, DC
BLANKET ORDERING AGREEMENT (BOA) N40080-19-R-2018
SUBMITTAL REQUIREMENTS
Upon submission of the following information, the government will review the documents and the firm will be determined to be either ELIGIBLE or INELIGIBLE. To qualify for receipt of a BOA, each firm must MEET
ALL the following submission criteria. If a firm fails to meet any factor, they will not be given a BOA. It is the responsibility of the firm to demonstrate they meet the government requirements and to ensure their submission is accurate and complete. The government does not contemplate seeking clarification or additional information.

____ Cover letter to include the following:
Your company name, physical address, and bona fide place of business address (if applicable).
Names, titles, telephone/facsimile numbers, e-mail addresses, and signature of person(s) authorized to negotiate and sign the agreement and future orders.
Your six digit primary NAICS Code as indicated on your Small Business Administration
8(a) participation letter.
Your firm’s Tax Identification Number (TIN).
The name of your servicing SBA representative, phone number, and e-mail address.
Your firm’s DUNS number. If you are using a bona fide place of business for the purposes of this BOA, please list the DUNS number for the bona fide place of business.
____ Copy of your 8(a) Certification Letter received from the U.S. Small Business Administration, which includes your servicing office and primary NAICS Code assigned. Firms may also select one secondary NAICS if they wish to be considered for additional work outside of their primary NAICS (see BOA Information Form). Only the following designated NAICS codes pertain to this BOA:

Only submission packages from construction firms in the “Developmental Stage” (first four (4) years of the
8(a) program) with a servicing office or a bona fide place of business in one of three Small Business
Administration (SBA) Districts (Washington, D.C.; Baltimore, MD; and Richmond, VA) will be considered.
Firms must still be certified as an 8(a) Developmental Stage participant at time of award (planned award date is February 1, 2019).
If your firm is not serviced by one of the SBA Districts in Washington, D.C., Baltimore, MD; or
Richmond, VA, your firm must have an SBA-approved bona fide place of business in one of these three SBA
Districts. In addition to the 8(a) certification letter, you must include a copy of the SBA letter approving your firm’s bona fide place of business. The bona fide place of business must be SBA-approved prior to the submission deadline indicated above in accordance with the SBA 8(a) Business Development Program
Compliance Guide.

____ First page of your record from the SBA Dynamic Small Business Listing as proof of your registration;
status should show “Active”. If your firm has a bona fide place of business, the SBA Dynamic Small Business
Listing must reflect the location of the bona fide place of business.
____ Print out of your “Entity Overview” from the System for Award Management (SAM) database https://www.sam.gov/portal/public/SAM/ as proof of your SAM registration. You can locate the Entity Overview screen by searching on your DUNS number in the Search Records area on the Home Page. The results of the search should include your firm’s information. Click on the “View Details” button included with the results. The
Entity Overview link is found under your “Entity Dashboard” on the left side of the screen. The Registration Status field should indicate “Active”. Verify your firm’s current physical address is properly recorded on your “Entity
Record”. A link to your Entity Record is found under the Entity Overview link. If your firm has a bona fide place of business, the Physical Address shown under your firm’s Entity Record must reflect the physical address of the bona fide place of business.
____ First page of your Representations and Certifications from the System for Award Management (SAM) database https://www.sam.gov/portal/public/SAM/ as proof of registration. A link to your “Reps and Certs” is found under the Entity Record link. If your firm has a bona fide place of business, the Representations and
Certifications record must reflect the DUNS of the bona fide place of business.
____ A recent letter (dated on or after the date of this posting) from a U.S. Department of the Treasury
Certified bonding company certifying your maximum bonding capacities (both per contract and aggregate capacities). Please refer to http://www.fiscal.treasury.gov/fsreports/ref/suretyBnd/c570_a-z.htm for the Department of the Treasury’s Listing of Certified Companies.
____ A copy of your firm’s state contractor’s license.
____ Evidence of submission of your VETS-4212 Report (confirmation of receipt from the United States
Department of Labor) as required by FAR 52.222-37, Employment Reports on Veterans. If you have received a contract either as the prime contractor or subcontractor from the federal government you may be subject to the VETS-4212 reporting requirement. Please refer to http://www.dol.gov/vets/vets4212.htm or call the helpdesk at (866) 237-0275 to determine if your firm is subject to this requirement. If this requirement is not applicable to your firm, provide a statement that your firm does not have any contracts subject to this requirement.
____ One (1) BOA Information Form for a project under your primary NAICS Code (indicated in your 8(a) certification letter) describing your firm’s technical experience/past performance on a project similar in size and general scope for which your firm would like to be considered. A project which primarily demonstrates your firm’s management/oversight capabilities does not adequately demonstrate your firm’s technical or trade experience. The project shall be ongoing or completed within the last three (3) years and must fall within the
