About this file

This is a Quality Assurance Surveillance Plan (QASP) Matrix for vacuum recovery unit rental services at Portsmouth Naval Shipyard. The document establishes performance standards and acceptable quality levels (AQL) for seven key deliverables: technical support, defective equipment repair, equipment start-up and commissioning, preventative maintenance, equipment delivery condition, personnel training, and preventative maintenance planning. Each requirement mandates 100% compliance with zero deviations unless expressly approved by PNSY, with response timeframes including 12-hour technician arrival for equipment repairs and 24-hour repair or replacement completion. Surveillance methods include validated customer complaints, user surveys, periodic inspections, and observation of delivered equipment, with failures resulting in Contract Discrepancy Reports (CDR) and potential payment impacts.

The QASP incorporates a five-tier performance rating system—Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory—to assess contractor performance against contractual requirements. Satisfactory performance, defined as the expected baseline, requires meeting all contractual requirements with only minor non-conformances and timely corrective actions. Higher ratings reward flawless execution with proactive issue management and ahead-of-schedule delivery, while lower ratings reflect significant failures, incomplete documentation, critical deficiencies, or failure to provide required operational and quality evaluation (OQE) data that jeopardize project success.

RatingsDescriptive Rating System
ExceptionalPerformance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule.
Very GoodPerformance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review.
SatisfactoryPerformance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time.
MarginalPerformance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight.
UnsatisfactoryPerformance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project.
