4. RESPONSIBILITITES
4.1. Government
4.1.1. N/A
4.2. Contractor
4.2.1. The equipment will be operating around-the-clock under a rigorous deployment performance schedule. Within four (4) hours of initial notification of the need for repair, the Contractor shall have a qualified and equipped technician on the job-site fully prepared to repair the rental equipment. The defective unit(s) shall be repaired within twenty-four (24) hours (one day) of initial notification or, if the original defective unit(s) cannot be made operational, replaced within forty-eight (48) hours (two days) of initial notification. If the equipment failure is not the fault of the Government, the Contractor shall bear all costs associated with its repair and/or replacement.
4.2.2. Government intended use is for submarine work that will include high pressure water blasting of outside hull. Government must be able to install a temporary pressure washer mount in the basket and a hose guide along the boom of each rented unit. (Modifications have been approved by JLG Industries Inc., (Case # 00556132). Before they are returned, the government will completely remove all modifications and ensure all boom lifts are in the same condition as received. The government will be responsible for any damage caused by the temporary modifications. Rental will be on a firm fixed price regardless of the hours used.
4.3. Maintenance
4.3.1. The contractor shall maintain a stringent maintenance program in accordance with Manufacturers’ recommendations, applicable American National Standard (ANSI) specifications and Scaffolding Industry Association (SIA) specifications. Except for daily checks of fluid levels, the Contractor is responsible for all equipment preventative maintenance (PM) and repairs for the duration of the lease. The Contractor shall coordinate with the Contracting Officer's Representative (COR) and the Temporary Service Zone Manager (TSZM), in writing, to schedule preventative maintenance.
4.3.2. The unit shall have no visible fluid leaks during operation or sitting idle.
4.3.3. Upon completion of regularly scheduled PMs, contractor will leave a copy of the completed maintenance/inspection record with the point of contact. All preventive maintenance/inspection records must clearly show what maintenance/inspection has been performed and must be signed and dated by the individual who performed the maintenance/inspection. A statement or certification to the effect that maintenance has been performed will not suffice. The record must reveal the actual elements of inspection and maintenance being performed.
4.3.4. Up to date inspections will be kept with the equipment at all times. It is the sole responsibility of the contractor to ensure preventive maintenance on all boom lifts is kept up to date. If records are not up to date and records are not provided as set forth in these specifications, the equipment will be inoperable to the government, and the government will not pay for the downtime caused by inadequate or incomplete records.
4.3.5. The contractor shall provide forty-eight (48) hour notification to the COR and the TSZM prior to providing preventative maintenance of the boom lift on site.
4.3.6. During the course of the rental period, the MEWP will be swapped out for a unit meeting the same specification if in the judgement of the Government there is an unacceptable frequency of breakdowns.
4.4. Training
4.4.1. N/A
4.5. Required Documents - Documentation will be located within a waterproof case affixed to each machine. Minimally, the documentation shall consist of:
4.5.1. An operator’s manual which contains specific operation/safety requirements that need to be followed for the applicable model MEWP.
4.5.2. A copy of the ANSI Manual of Responsibilities.
4.5.3. Signed/dated hard copy documentation showing the current annual and subsequent frequent inspections (quarterly) if the annual is more than 3 months old.
The vendor's inspection sticker or date stamp on the machine is not acceptable documentation for the Navy, and absence of current hard-copy inspection documents renders the unit inoperable to the Navy. New equipment acceptance will be delayed, and previously accepted machines will be rendered out of service if current and accurate paperwork is not provided. The government will not be billed for time periods that the unit is out of service and the invoice will be adjusted accordingly at no cost to the government.
4.5.4. Lifting & Handling instructions and lift-point/padeye certification.
4.5.5. Points of contact with telephone numbers for requesting emergency repair regardless of the time or type of day.
5. DELIVERY / RETURN
5.1. Delivery - The Contractor shall deliver machinery that is fully-charged/fueled, fully-assembled, ready-to-use, and in a first-class-operating condition and functions as intended. Machines shall be functionally tested prior to delivery. The machinery with its support equipment shall be delivered to the site on or prior to the request date. The Contractor shall confirm the date and time of delivery prior to each delivery with the COR and TSZM, if appointed. Delivery truck(s) will be capable of loading/unloading the unit(s) at ground level without a loading dock or additional material handling equipment. The Contractor shall provide the COR and TSZM with an itemized list of all equipment delivered at the time of delivery.

5.3.1. The equipment shall be delivered in fully functional, safe operating condition.
The contractor will deliver equipment with the aforementioned certification records, manuals, and other documentation listed in this SOW.
5.3.2. A joint inspection of each lift shall be performed by the Contractor and a shipyard licensed operator using enclosure (2) of NAVSHIPYD PTSMHINST 11262.2 before the unit is accepted by the Government. If any requirements listed in this SOW are lacking or deficient, the equipment acceptance will be delayed, and the invoice will be adjusted accordingly. The rental period does not begin until the government has accepted the boom lift. The contractor shall notify the COR or Technical Point of Contact a minimum of 48 hours prior to delivery of units.
5.3.3. Equipment must meet all criteria defined in this specification.
5.4. Packaging and Marking
5.4.1. N/A
