About this file

This is an Offeror's Past Performance Data questionnaire for solicitation N00024-26-R-S005, issued by NAVSUP FLC San Diego. The form is designed to collect references and performance evaluations from previous clients or contracting officers who have worked with a prospective contractor on past contracts. Completed questionnaires must be returned to Jason Pelle at NAVSUP FLC San Diego (phone: 619-556-9676, email: jason.a.pelle.civ@us.navy.mil) no later than the solicitation closing date.

The questionnaire evaluates contractors across four primary performance dimensions: (1) Quality and Meeting Contract Requirements, including customer satisfaction, rework instances, and innovative applications; (2) Quality and Timeliness, assessing schedule compliance and timely submission of reports and invoicing; (3) Contractor Responsiveness, examining the ability to address customer concerns, resolve problems, and maintain cost control; and (4) Additional Observations, requesting identification of contractor strengths and weaknesses, overall satisfaction with contractor performance, awareness of similar contracted efforts, and recommendations for additional references. Respondents are instructed not to return the form to the originating contractor and are encouraged to provide detailed explanations for any deficiencies or contract value changes, with additional pages available if needed for expanded commentary.
