5.0 PROPOSAL ORGANIZATION
VOLUME I: PAST PEFORMANCE (FACTOR 1)
Past performance information shall be provided for the Prime and all subcontractors in order to develop an Offeror's past performance rating.
Past performance information is one indicator of an Offeror’s (to include prime and subcontractors) ability to perform the contract successfully. Using the Past Performance Matrix (as listed in Section J), the Offeror and all subcontractors shall provide a separate list of relevant contracts that were performed within the last three years.
Contracts that are listed in PPIRS are preferred. The list for the Prime shall contain no more than seven relevant contracts. In addition, each subcontractor shall complete a separate matrix and provide no more than seven relevant contracts. The identified relevant contracts shall focus on performance that is relevant to the current solicitation requirement of this solicitation. Relevancy includes similarity of item, complexity and dollar value.
Evaluation of past performance will primarily be based upon the information submitted by the Offeror through its
Past Performance Questionnaires. There are two aspects to the past performance evaluation: (1) First, the
Government will evaluate whether the Offeror’s past performance is relevant or not relevant to the solicitation requirement; (2) Second, for those efforts considered relevant, the Government will then determine how well the
Offeror performed on each effort. If the Offeror does not have a record of relevant past performance, information on past performance is not available, or no assessment of how well the Offeror performed can be done, then the Offeror shall be determined to have unknown past performance. Unknown past performance will be considered
“Acceptable.”
The Government will evaluate the Offeror’s relevant past performance information only during the last three years, in accordance with Section L. Information will be gathered in the following areas: (1) customer satisfaction,
(2) timeliness, (3) technical success, (4) program management, and (5) quality. The reference information must be current to facilitate the evaluation process. If the Offeror has no relevant past performance within the last three years or cannot provide a list of relevant contracts, then the Offeror must provide an explanation.
The Government also reserves the right to consider past performance information available from other sources, including, but not limited to, information in the Contractor Performance Assessment Reporting System (CPARS), the Past Performance Information Retrieval System (PPIRS) (http://www.ppirs.gov/), and the Federal Awardee
Performance and Integrity Information System (FAPIIS).
The following definition is provided for the past performance rating:
Acceptable: Based on the Offeror’s recent and relevant performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror’s performance record is unknown.
Unacceptable: Based on the Offeror’s recent and relevant performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
Past Performance Questionnaires (as listed in Section J) only apply to relevant contracts for which performance information is not available through PPIRS. The Offeror and subcontractors shall submit the Past Performance
Questionnaire to all of their references listed on the Past Performance Matrix that have NO PPIRS contracts, and shall request the references to complete the Past Performance Questionnaire and return it directly to:
Contracts Department
NSWC Indian Head Explosive Ordnance Disposal Technology Division
Building 1558
4072 North Jackson Road
Suite 124, Code 022SL
Indian Head, MD 20640-5115
Surveys may also be emailed to stclair.larry@navy.mil which is the preferred method of delivery.
REMINDER: It is imperative that Offerors label all price files with Volume III, company name and a unique identifying name which is reflective of the contents of the file.
VOLUME II – TECHNICAL (FACTOR 2)
The Offeror shall address technical subfactors as detailed below which are of equal language.. Offerors are to ensure that only Technical information is contained within Volume II. The resultant Offeror's proposal will be incorporated into the contract by reference and the Offeror shall perform in accordance with the technical proposal presented with its proposal.
The technical proposal shall be limited to the following submissions and pages:
Subfactor 1: Technical Approach to Statement of Work (Not to Exceed 20 pages)

Offeror shall demonstrate technical soundness in their approach to performing and delivering the applicable requirements within the specified timeframes. The Offeror shall provide a thorough description of the manufacturing plan proposed for production including the description of each operation and procedure as well as production rate capabilities. The proposal shall demonstrate technical expertise in the areas of metal stamping, machining, and plastic molding. All subcontracted work (if any) shall be discussed in the same level of detail as the in-house production.
mailto:stclair.larry@navy.mil
The Offeror shall identify any potential problem areas or risks of the manufacturing process operations shown in the proposal (i.e. plastic molding, metal stamping, machining). The Offeror shall also describe, in detail, how they propose to mitigate these risks. The proposal shall include detailed facility layout and utilization of floor space to be used throughout the contract. The layout shall include the space designated and process flow for all production equipment, material/product handling and storage, inspection, packaging and shipping.
The Offeror shall provide individual qualifications, training of all key personnel who shall be responsible for executing their proposed manufacturing/fabrication plan.
The Offeror shall provide a fully detailed schedule identifying major tasks and events to meet the RFP requirements.

The Quality Manual must demonstrate that the offeror's quality system meets the Higher Level Contract Quality Requirements. The offeror’s quality system must also provide information on the following areas.
Organizational structure, responsibilities, procedures, process control methods and techniques, and resources proposed for implementing its quality management system (both internal and flow-down to its suppliers).
Supplier evaluation and control program that demonstrates its ability to evaluate suppliers, provide pertinent requirements to control supplier product, and manage quality, including non-conforming material procedures for handling of defective material, rework, and repair parts for both in-house and at subcontractors' facilities.
Description of the ISO 9001 2008 program that will be used to meet the requirements of the solicitation and how it will be implemented.
VOLUME III – PRICE (FACTOR 3)
This volume is the only volume that shall contain price information. There is no limitation on the number of pages or print size for the price proposal. All Attachments submitted under Volume III (Factor 3) shall be clearly identified.
The Price Volume shall include the completed RFP document, with Section B, pricing filled in. The Price Volume shall include any available pricing information to facilitate the price analysis that will be performed in evaluating the proposal (i.e., price breakdown, catalog pricing, past pricing history, etc.).
The price information shall include data regarding the general financial condition of the offeror and specific plans for financing the proposed contract. The Government has no plans to provide any financial assistance.
The Price Volume shall also include all executed representation certifications. The Offeror shall not take any exceptions or make any deviations to the terms of the RFP, as any exception or deviation in a proposal will be considered to be a deficiency.
IHEODTD 211 - SECTION M BEST VALUE EVALUATION AND BASIS FOR AWARD (FFP)
(NAVSEA/NSWC IHEODTD) (OCT 2013)
General:
1. It is the intention of the Navy to award up to three, Indefinite Delivery, Indefinite Quantity (fixed price supply) contracts with one base year and four option years.
2. Award will be made to that Offeror whose proposal is determined to be technically acceptable at the lowest price.
Offerors are required to follow specific instructions in submitting their proposal. Each Offeror’s submission will be screened upon receipt by the Contracting Officer or a designee of the Contracting Officer to ensure compliance with the RFP instructions and requirements. Elimination of an Offeror due to failure to comply with the submission requirements of the RFP is at the sole discretion of the Contracting Officer.
3. As a result of this solicitation, the Government intends to award based on initial offers and without holding discussions, but reserves the right to hold discussions at the Government’s discretion. Therefore, the Offeror’s initial proposal should contain the Offeror’s best terms from a price and technical standpoint. However, in accordance with FAR 15.306 -- Exchanges With Offerors After Receipt of Proposals, may occur when award without discussions is contemplated. IAW FAR: “Clarifications are limited exchanges, between the Government and
Offerors... If award will be made without conducting discussions, Offerors may be given the opportunity to clarify certain aspects of proposals (e.g., the relevance of an Offeror’s past performance information and adverse past performance information to which the Offeror has not previously had an opportunity to respond) or to resolve minor or clerical errors.”
4. The following factors will be used to evaluate offers.
Volume I: Factor 1: Past Performance
Volume II: Factor 2: Technical: Subfactors listed are of equal importance.
Subfactor 1 –Technical Approach to Statement of Work
Subfactor 2 –Quality Assurance
Volume III: Factor 3: Price
Factor 1 – Past Performance (Volume I)
The Government will evaluate the Prime’s and Subcontractors’ past performance to develop the Offeror’s past performance rating. Past Performance will be evaluated by the Contract Specialist.
The Offeror shall provide up to three references listed on the past performance matrix (Attachment to Section J) for federal government contracts for supplies performed in the last three years from the date of issuance of the solicitation that are the same or similar to those required under this procurement. If the offeror does not have any
Federal Government contracts, then list state, local, or commercial contracts, in that order. Failure to submit any
Contractor Past Performance Questionnaires shall be considered certification that the contractor has no record of relevant and meaningful past performance for the Government to evaluate. In that case, the offeror shall affirmatively state that it possesses no relevant past performance and explain why it has no relevant past performance or cannot provide any Questionnaires. Failure to provide the required information or to provide an explanation may result in the removal of the Offeror from consideration for award.
Relevant past performance is defined as experience that is of the same or similar scope (i.e, for the same or similar items), of the same or similar magnitude, and of the same or similar complexity to that which is described in the solicitation.
The Past Performance Questionnaire includes questions on (1) customer satisfaction; (2) timeliness; (3) technical success; (4) program management; and (5) quality. The Government will not assign a rank to the Past Performance factor, but will determine the acceptable or unacceptable nature of the past performance for each Offeror based on the information provided and review of other sources of past performance information available to the Government.
Factor 2 – Technical (Volume II)
The Government has established the page limit for each technical subfactor in Section L of the RFP. Offerors are advised that excess pages will not be evaluated. The Government will assess whether or not the Offeror has simply restated the Statement of Work, which is unacceptable.
Sub-Factor 1: Technical Approach to Statement of Work (Not to Exceed 20 pages)
The Government will evaluate the completeness and thoroughness of the Offeror's proposed plan to fabricate, inspect, test, and deliver the MK 117 components in accordance with all applicable documents, including the drawing package and requirements of the RFP. The Government will evaluate the Offeror’s proposal to determine the extent of innovation and the technical soundness of the proposed approach to performing and delivering the applicable requirements within the specified timeframes.
The Government will evaluate the demonstrated capability of Offeror’s manufacturing plan for the production of this cartridge including the description of each operation and procedure as well as production rate capabilities. The

SECTION M - EVALUATION FACTORS FOR AWARD
HQ M-2-0016 USE OF PAST PERFORMANCE INFORMATION RETRIEVAL
SYSTEM – STATISTICAL REPORTING (PPIRS-SR) IN PAST PERFORMANCE
EVALUATION (NAVSEA) (SEP 2009)
(a) Each offeror’s past performance shall be evaluated in accordance with FAR 13.106-2 or
15.305(a)(2), as applicable. The Department of Defense (DOD) has authorized use of PPIRS-SR for use by DOD activities during the acquisition of supplies and services. For purposes of this solicitation, contractor past performance will be based on data from PPIRS-SR. The offeror’s quality and delivery classifications from PPIRS-SR will be used in conjunction with the offeror’s references, the criteria in FAR 13.106-2 or 15.305(a)(2), as applicable, and other provisions of this solicitation.
(b) The purpose of PPIRS-SR is to provide source selection officials with quantifiable past performance information regarding delivery and quality. This information is collected form existing DOD reporting systems during the source selection process.
(c) Quality: PPIRS-SR collects quantifiable quality data from existing systems and uses that data to classify supplier performance by Federal Supply code or federal Services Code (FSA group).
Based on comparisons among suppliers in a specific FSC group, PPIRS-SR sorts suppliers into color ratings representing the supplier’s overall quality performance based on the following indices:
Color Position
Dark Blue Top 5% of suppliers in FSC group
Purple Next 10% of suppliers in FSC group
Green Next 70% of suppliers in FSC group
Yellow Next 10% of suppliers in FSC group
Red Bottom 5% of suppliers in FSC group
(Note: if all supplier ratings for a specific FSC group are equal, all suppliers with that group will be classified green. Suppliers with no history in PPIRS-SR will be displayed without a rating and, for evaluation, receive a neutral rating.)
(d) Delivery: supplier deliver past performance is classified in PPIRS-SR by the suppliers percentage of on-time deliveries. On-time deliveries are calculated using the number of line items delivered and a weighting factor reflecting the length of time a delivery was overdue.
(e) PPIRS-SR classifications are determined monthly for each supplier and can be reviewed at http://www.ppirs.gov/.
