About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Public Health (IDPH) for the procurement of microfilm scanners and related services. The RFQ specifies the required features for the microfilm scanners, including compatibility with 16mm x 100 ft Fuji microfilm, the ability to view and download files to a USB drive, and connection to a printer and personal computer. The RFQ also requires the provision of OCR software and a five-year service agreement for on-site repair, maintenance, and customer support. The RFQ requests quotes for four line items, with a requirement for vendors to provide quotes for all items to be deemed responsive. The RFQ also outlines COVID-19 protection requirements for vendors, based on recent Executive Orders issued by the Illinois Governor.
The RFQ notes that the resulting contract will be a one-time purchase, with the winning vendor required to possess all mandatory qualifications and offer the lowest bid. The RFQ also indicates that financial disclosures are required if the value of the resulting contract is over $50,000, and that vendors registered on the Illinois Procurement Gateway should complete and submit specific forms. Additionally, the RFQ states that small businesses awarded a contract procured as a small business set-aside may subcontract no more than 49% of the value of the contract to a business not registered in the Small Business Set-Aside Program.
