About this file
This document is a Project Manual and Asbestos-Containing Building Materials (ACBM) Inspection Report for the Metrocare Pharmacy renovation project located at 9696 Skillman Street, Dallas, Texas 75243. The project involves the conversion of an existing 3,490 square foot office suite into a Class A closed pharmacy facility. Metrocare is soliciting formal proposals from General Contractors for full turn-key renovation services, including labor, materials, debris removal and disposal, and all necessary permitting. The preconstruction site walkthroughs are scheduled for Friday, January 23, 2026 at 1:00 p.m. C.T. and Monday, January 26, 2026 at 1:00 p.m. C.T., with construction anticipated to commence in March 2026. Metrocare facilities operate Monday through Friday from 8:00 a.m. to 5:00 p.m. The project scope includes comprehensive construction specifications across multiple divisions addressing concrete, carpentry, thermal and moisture protection, openings, finishes, specialty items, fire suppression, plumbing, HVAC, electrical systems, communications, and electronic safety and security systems designed to ensure structural integrity, energy efficiency, and compliance with healthcare facility regulations.
The May 9, 2025 asbestos inspection identified several asbestos-containing materials within the first floor renovation areas that require management prior to construction activities. Category II non-friable asbestos products were detected, including green adhesive associated with green marble wall base (2% chrysotile, 160 SF), beige sink undercoating (5% chrysotile, 5 SF), and grout and mortar associated with green marble flooring (presumed asbestos, 650 SF). The inspection encompassed 150 bulk material samples analyzed using Polarized Light Microscopy (PLM) with dispersion staining by Moody Labs, an NVLAP-accredited laboratory. Per Texas Asbestos Health Protection Rules (TAHPR), only licensed Asbestos Abatement Contractors may remove, repair, enclose, or encapsulate asbestos-containing materials, and all abatement activities must be monitored by a licensed Asbestos Consultant. The Texas Department of State Health Services must be notified at least 10 working days before initiating abatement activities. The proposal pricing shall encompass all costs associated with managing identified asbestos materials in compliance with NESHAP regulations and TAHPR requirements.

INSPECTION REPORT
Commercial Office Building 9696 Skillman Street Dallas, Texas 75243
May 9, 2025
I. INTRODUCTION
Farmer Environmental Group, LLC (Farmer) inspected the First Floor of the Commercial Office Building (the Site) located at 9696 Skillman Street in Dallas, Texas for asbestos-containing building materials (ACBM) on April 30 & May 1, 2025. Texas Department of State Health Services (TDSHS)-licensed Asbestos Inspector John Purcell (license #603293) conducted this inspection under an agreement between Farmer and Metrocare Services. The scope of work included all suspect accessible interior materials on the first floor that may be affected by future renovation activities at the site. Exterior & roofing materials were specifically excluded from the scope of work.
The purpose of the inspection was threefold: 1) determine locations and conditions of asbestos-containing products used at the Site; 2) determine the impact these materials will have on planned renovation activities; and 3) assist Metrocare Services and/or their client in obtaining the necessary building permit with the City of Dallas in accordance with Senate Bill 509.
II. SUMMARY OF FINDINGS & RECOMMENDATIONS
Based on the laboratory analysis of the bulk samples collected, the following asbestos products were identified at the Site:
• Green adhesive associated with green marble wall base
• Beige sink undercoating
• Grout & mortar associated with green marble flooring (presumed)
Farmer was unable to sample the green marble flooring in the main lobby without causing significant damage to the flooring. Therefore, the flooring grout and mortar should be presumed to contain asbestos until testing becomes feasible.
It should be noted that the inspector made reasonable attempts to determine whether suspect flooring applications were present beneath existing flooring materials. However, suspect asbestos-containing flooring materials may be present under existing flooring materials that were inaccessible without significantly damaging floor covering. Farmer has extensive experience with commercial structures that have been abated prior to demolition or renovation projects. Because commercial renovations occur from time to time, existing asbestos-containing floor mastics are occasionally found in sporadic areas under new flooring materials. Although representative samples of existing flooring materials were collected and analyzed for asbestos content during the asbestos inspection, an asbestos inspector cannot confirm that no suspect flooring mastics exist in other areas without removing all the existing flooring materials. Therefore, demolition of flooring materials should proceed with caution. If suspect flooring materials not identified in this report are uncovered during demolition activities, flooring material demolition should cease until samples of the suspect floor material are analyzed for asbestos content.
Beneficiaries of this report should be aware that Farmer did not have permission to conduct destructive testing within the structure at the time of our assessment.
As a result, no demolition for access was conducted behind wet walls, columns, hard ceilings, etc. Farmer made reasonable efforts to access all materials without damaging building finish-out. Destructive testing should be conducted prior to any demolition or renovation activities that may impact interstitial wall spaces, columns, hard ceilings, or otherwise concealed materials.
Additional bulk samples should be collected from previously concealed materials that may be revealed during demolition activity, or from materials outside the original inspection scope of work. "Suspect" material includes any material serving as a sprayed-on or troweled-on acoustic or fireproofing surface; floor and ceiling tiles, cement asbestos panels, siding, or shingles; thermal insulation or any material associated with mechanical systems; or any binding agent such as tar sealant, mastic adhesive, roofing tar, caulking, etc. TDSHS regulations require bulk samples in public buildings be collected by a licensed asbestos inspector or individual asbestos consultant.
III. SAMPLE RESULTS
ASBESTOS AND NON-ASBESTOS PRODUCTS IN THE FACILITY
The PLM Report in Appendix I shows the results of PLM analysis for bulk samples collected at the Site, as required under Task 1 above. The PLM Report shows the room or area in which the sample was collected and provides a description of the sampled material. Site drawings are included in Appendix II.
Any product containing greater than 1% fibrous asbestos, when analyzed by PLM, is considered a potential hazard by the United States Environmental Protection Agency (US EPA). However, it should be noted that the Texas Asbestos Health Protection Rules (TAHPR) lowered the threshold level in Texas from ‘greater than one percent asbestos’ (current US EPA guideline) to ‘one percent or greater’ as of July 8, 2021. PLM is the US
EPA-recognized method for determining fibrous bulk asbestos content. Additionally, the US EPA indicates that further analysis by point counting be performed to confirm asbestos content for friable materials found to contain less than ten percent asbestos by visual area estimation. Point counting analysis may be omitted if the material is assumed to contain equal to or more than one percent asbestos and is then handled accordingly. All samples collected by Farmer were analyzed by Moody Labs, LLC, a TDSHS-licensed and National Voluntary Laboratory Accreditation Program (NVLAP)-accredited laboratory located in Farmers Branch, Texas. The samples were analyzed by PLM coupled with dispersion staining, in general accordance with US EPA-600/R-93/116. Utilizing this procedure, the various asbestos minerals (chrysotile, amosite, crocidolite, actinolite, tremolite, and anthophyllite) can be determined by the microscopist.
Sample analysis reported as "None Detected" should be interpreted as meaning no asbestos was observed in the suspect material above the reliable limit of detection, (Less Than 1% by visual estimation) for the PLM method.
Sample analysis reported as "Not Analyzed – Positive Stop" should be interpreted as meaning a sample of the same homogeneous material was analyzed with an asbestos content equal to or greater than 1%. Further analysis is therefore considered unnecessary.
The US EPA's National Emission Standards for Hazardous Air Pollutants (NESHAP) asbestos regulation (US EPA 40 CFR 61, Subpart M) classifies asbestos-containing materials into the following three categories:
Category I Category I Non-Friable ACM refers to asbestos-containing packing, gaskets resilient floor covering, and asphalt roofing products containing more than 1% asbestos that, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
Category II Category II Non-Friable ACM is any non-friable material that is not Category I and contains greater than 1% asbestos that, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
RACM Regulated Asbestos-Containing Materials (RACM) is a friable (reducible to powder by hand pressure) manufactured ACM; or a Category I non-friable ACM that has become friable; or a Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading; or Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations. RACM should be removed prior to renovation or demolition activities.
RACM is further defined by NESHAP as:
a) Friable asbestos material;
b) Category I nonfriable ACM that has become friable;
c) Category I nonfriable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading, or
d) Category II nonfriable ACM that has become crumbled, pulverized, or reduced to powder or has a high probability of becoming crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations regulated by NESHAP asbestos regulation (US EPA 40 CFR 61, Subpart M).
The following category II non-friable asbestos products (materials not reducible to a powder or dust form under normal hand pressure) were identified at the Site:

IV. RECOMMENDATIONS
Based on observations made at the Site, and results of the analysis of bulk samples collected during the inspection, Farmer recommends the following:
1. TAHPR dictates that only a licensed Asbestos Abatement Contractor can remove, repair, enclose, or encapsulate asbestos containing material. TAHPR further dictates that the asbestos abatement activity be monitored by a licensed Asbestos Consultant.
2. Green marble flooring should be tested for the presence of asbestos-containing grout & mortar prior to renovation activities that will impact these materials.
3. According to TAHPR, TDSHS must be notified of any asbestos abatement of a structure regardless of size. Notifications must be postmarked at least 10 working days before initiating abatement activities and must be made using TDSHS forms.
Forms can be obtained by contacting the TDSHS Asbestos Notification and Information Section at 1-800-572-5548 or online through the TDSHS website at www.dshs.state.tx.us.
4. To comply with National Emission Standards for Hazardous Air Pollutants (NESHAP) asbestos regulation (EPA 40 CFR 61, Subpart M), Farmer recommends removing all friable and non-friable ACBM that has the potential to become friable under abatement conditions prior to renovation or demolition activities.
5. Additional bulk samples should be collected from previously concealed materials that may be revealed during demolition activity, or from materials outside the original inspection scope of work. "Suspect" material includes any material serving as a sprayed on or troweled on acoustic or fireproofing surface; floor and ceiling tiles;
transite panels, siding, or shingles; thermal insulation or any material associated with mechanical systems; or any binding agent such as tar sealant, mastic adhesive, roofing tar, caulking, et cetera. TDSHS regulations require bulk samples in public buildings be collected by a licensed asbestos inspector or individual asbestos consultant.
6. The building owner should be aware that removing ACBM does not discharge ownership. During transport or after burial in an EPA-approved landfill, any contamination throughout the lifetime of the material remains the responsibility of the building owner.
7. If any material cannot be positively identified as non-asbestos containing by the analytical results of samples collected during this inspection, it should be treated as asbestos until a sample of the newly identified suspect material can be collected and analyzed by PLM.
8. During renovation activities, the client should ascertain the contents of any new products (obtain Safety Data Sheets) installed in the building that would avoid the accuracy and validity of this inspection report.
9. According to the Occupational Safety and Health Administration (OSHA) 29 Code of
Federal Regulations (CFR) 1926.1101, building and/or facility owners shall notify the following persons of the presence, location, and quantity of asbestos-containing materials, at the work sites in their buildings. Notification either shall be in writing, or shall consist of a personal communication between the owner and the person to whom notification must be given or their authorized representative:
a) Prospective employers applying or bidding for work whose employees reasonably can be expected to work in or adjacent to areas containing such material;
b) Employees of the owner who will work in or adjacent to areas containing such material;
c) On multi-employer work sites, all employers of employees who will be performing work within or adjacent to areas containing such materials;
d) Tenants who will occupy areas containing such material.
V. LIMITATIONS
Farmer has endeavored to inspect the existing conditions within the affected areas using TAHPR inspection protocols. Regardless of the thoroughness of an inspection, it is possible that some areas containing asbestos were overlooked or inaccessible. This report presents general descriptions of various construction materials and the general locations where these materials were encountered. If questions arise during the planning for renovation, demolition or other construction, Farmer should be notified to permit a review of the situation and to present recommendations.
As noted above, beneficiaries of this report should be aware that Farmer did not have permission to conduct destructive testing within the structure at the time of our assessment.
This report has been prepared on behalf of and exclusively for the use of Metrocare Services and their client. The conclusions expressed by Farmer regarding the conditions of the site are based solely on the observations made on April 30 & May 1, 2025, the data collected during this inspection, and the laboratory results of the samples collected and analyzed.
The beneficiaries are hereby advised that conditions observed are subject to change.
This report and the findings contained herein shall not, in whole or in part, be disseminated or conveyed to any other party or be used or relied upon by any other party, in whole or in part, without Metrocare Services’ or Farmer's prior written consent.
