About this file
This document is a Justification and Approval (J&A) from the United States Special Operations Command (USSOCOM) for acquiring eight Philips IntelliVue X3 patient vital signs monitoring systems. The procurement is for the SOMEDD (Special Operations Medical Equipment Development Directorate) to support surgical care in austere environments, with specific requirements including a compact, lightweight, battery-operated monitor weighing 1.4 kg, capable of measuring ECG, SpO2, respiration, NIBP, invasive blood pressures, temperature, and end tidal CO2. The document details that the monitors will include various accessories such as reusable ECG cables, NIBP hoses, BP cuffs in multiple sizes, and temperature monitoring capabilities.
The J&A justifies the limited source procurement by highlighting the monitors' compatibility with existing equipment bags, the unit's familiarity with the device, and its superior accuracy in complex surgical cases. Market research confirmed three authorized resellers of the monitors through GSA Advantage, and the unit already possesses eight of these monitors, seeking to complete their equipment sets. The procurement aims to enhance the SOMEDD's ability to provide surgical care in challenging environments with a reduced equipment footprint, with the selected monitor being chosen for its size, weight, and interoperability with current equipment.

3. Description of Supplies/Services: (FAR 8.405-6(c)(2)(iii)) The supplier will provide eight portable, compact, lightweight battery-operated patient vital signs monitors. The product should weigh 1.4 kg and have 5 dynamic waveform technology with a durable touchscreen, and no carry handle. The monitor must be able to measure ECG, SpO2, respiration, NIBP, invasive blood pressures, temperature, and end tidal CO2. The cubic size should be 164 cubic inches. The monitor should come with a docking station for stationary operations with AC power cord. Should also have a smart battery charger to sustain battery life. Each monitor should come with re-usable ECG cables and connections for 5-lead measurement with snap on ECG electrode connection. Each monitor will also have an NIBP hose and reusable BP Cuffs in sizes: child, small adult, adult, adult extra-long, and large adult. Each monitor will also come with a temperature monitoring cord that allows connection to disposable temperature monitoring devices. The device should also come with an invasive blood pressure monitoring cable along with all quoted accessories. These products will allow the SOMEDD to care for multiple patients safely, as well as recharge and maintain the battery life of the monitors. The Philips IntelliVue X3 monitor fills all of the above requirements.
4. Authority Cited Permitting Limited Sources: (FAR 8.405-6(c)(1)(i)) (Identify the citation permitting limited sources. This section is normally the most detailed part of the justification. To assist you in preparing this justification, an introductory sentence for each of the FAR 8.405-6(a)(1)(i) circumstances for limiting sources are provided below.
SOMEDD has unique mission requirements to provide surgical care in austere environments and en-route casualty care with reduced footprint. Current fielded equipment restricts the SOMEDD from performing austere resuscitative surgical care ISO SOF peculiar missions and potential future A2/AD environments. The monitor was selected due to size and weight and it's compatibility with current equipment bags. Other similar monitors are not inter operable with the bags or have excess capabilities and cost beyond the selected monitor.
5. Efforts/Actions to Increase Competition: (FAR 8.405-6(c)(2)(viii)) Inquiries were put out to Philips Healthcare and was discovered they have two authorized resellers within GSA Advantage. Austere Resuscitative Surgical Care subject matter experts conducted market research on current commercially available monitors and determined the selected monitors met all the performance parameters based on unit requirements.
6. Market Research: (FAR 8.405-6(c)(2)(vi)) Market Research was completed via Internet search as well as GSA vendors via GSA Advantage returned with positive results. Currently there are three authorized resellers to include the manufacturer in GSA Advantage offering the required medical monitors and components.

8. Other Facts Supporting the Justification: (FAR 8.405-6(c)(2)(vii)) SOMEDD already has inventory totaling eight Philips X3 monitors and requires an additional eight to complete equipment sets. Other comparable medical monitors are not inter operable with unit surgical bags due to size and has excess capability that in not required for the unit's mission. The excess capability increases cube and weight for the monitor and associated diagnostics devices and consumables.
SOMEDD end-users have extensive training and experience with the selected monitor over other comparable monitors. The selected monitor has a higher degree of accuracy in complex surgical cases, therefore increasing patient survival rates during and post-surgery.
9. Technical and Requirements Certification: (FAR 8.405-6(c)(2)(x))
