4.1 Nonconformance of Lot Acceptance: In the event the LAT fails to meet requirements as described in Technical Requirements, the contractor shall submit plans for the corrective action or disposition to the USG for approval. Minor failures may be corrected during the testing, with
USG approval. Production shall not be initiated without USG approval. Failure analysis and corrective action procedures and reporting shall be done per paragraph 4.1.1.
4.1.1 Failure Reporting, Analysis, and Corrective Action System: The contractor shall use a closed loop failure reporting system, procedures for analysis of failures to determine the root cause, and documentation for recording corrective actions taken. The Failure Reporting, Analysis, and Corrective Action System (FRACAS) shall include uniform failure reporting, failure analysis reports and corrective actions. The contractor shall notify the USG of any failure impacting cost, schedule, and performance. All failures shall be reported as required to the USG for review. All failures shall be categorized as in-field or in-factory failures. Each FRACAS report shall, at a minimum, identify the root cause, and detail the remedial action taken. The
