SECTION I

INTRODUCTION

This quality assurance surveillance plan (QASP) is pursuant to the requirements listed in the performance work statement (PWS)/Statement of Work (SOW) entitled Special Missions Branch (SMB) Training Support Services. This plan sets forth the procedures and guidelines the Special Missions Branch, Special Operations Training Group, Camp Hansen, Okinawa will use in ensuring the required performance standards or services levels are achieved by the contractor/contracted instructors.

Purpose:

The purpose of the QASP is to describe the systematic approach and effective surveillance methods used to monitor contractor performance for each performance objective listed on the Performance Requirements Summary (PRS). The QASP provides a means for evaluating whether the contractor is meeting the performance standards/quality levels identified in the PWS and the contractor’s quality control plan (QCP), and to ensure that the government pays only for the level of services received.

This QASP defines the methodologies used to monitor and evaluate the contractor’s performance, describes quality assurance documentation requirements, and describes the analysis of quality assurance monitoring results.

Performance Management Strategy:

1) Contractor’s QC Program: The contractor is responsible for the quality of all work performed. The contractor measures that quality through the contractor’s own quality control (QC) program. QC is work output, not workers, and therefore includes all work performed under this contract regardless of whether the work is performed by contractor employees or by subcontractors. The contractor’s QCP will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the PWS. The contractor will develop and implement a performance management system with processes to assess and report its performance to the designated government representative. The contractor’s QCP will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the PWS. This QASP enables the government to take advantage of the contractor’s QC program.

2) Government’s Quality Assurance Surveillance:

The government representative(s) will monitor performance and review performance reports furnished by the contractor to determine how the contractor is performing against communicated performance objectives. The government will make determination regarding incentives based on performance measurement metric data and notify the contractor of those decisions. The contractor will be responsible for making required changes in processes and practices to ensure performance is managed effectively. Any nonconformance with contract requirements is considered a “defect”. The term defective is used in reference to a service output that does not meet the standard performance specified in the contract.

Revisons to the QASP:

Revisions to the QASP are the joint responsibility of the Contracting Officer’s Representative (COR), Quality Assurance Personnel (QAP), Contracting Officer (CO), and the Quality Assurance Program Coordinator. The government intends to adjust, as warranted throughout the life of the contract, the level of government surveillance required commensurate with the contractor’s level of satisfactory performance.

SECTION II

SURVEILLANCE OF CONTRACTOR INSTRUCTOR PERFORMANCE

1) INSTRUCTION:

SURVEILLANCE METHOD: Instructors will be certified utilizing the Instructor Evaluation Checklist. Certification will be maintained through quarterly Observation, Environment, and Safety Review Checklists as well as Instructor Rating Forms filled out by the students for each period of instruction/class.

STANDARD: The checklists evaluate the instructor’s abilities in multiple areas. After completion of each observation a panel consisting of the Academics Chief, Section Lead, and headquarters element will discuss the instructor deficiencies. At this time the panel will decide whether the instructor is qualified to either start teaching or continue teaching students or whether they must remediate. If the instructor is unable to be certified in three attempts or shows a pattern of poor performance requiring multiple sessions of remediation after being certified, the COR will be informed of the requirement to replace the instructor.

SURVEILLANCE PROCEDURE: Instructors will be certified on each period of instruction/class by, at a minimum, the Section Leader and the Academics Chief before being put in front of students. The instructor will be graded with the Instructor Evaluation Checklist. Once certified, an instructor will be evaluated at a minimum of once per quarter utilizing the Observation, Environment, and Safety Review Checklists during instruction to the students. 10% of students will also fill out Instructor Rating Forms for each period of instruction/class.

2) GENERAL BEHAVIOR

SURVEILLANCE METHOD: Quarterly Progress Reports will be conducted by the Section Leader or Headquarters Element to address deficiencies and acknowledge positive actions by the instructor.

STANDARD: Any discrepancy that is identified during the monthly progress reports must be addressed and fixed by the instructor before the next counseling. Continuous negative progress reports, or if a contracted instructor refuses to address and fix a discrepancy it will require the COR to be informed of the requirement to replace the instructor.

SURVEILLANCE PROCEDURE: Instructors will constantly be observed by other instructors and the headquarters element of the branch. Any issues or concerns with the instructor’s performance or conduct will be addressed on a quarterly base.

SECTION III

UNACCEPTABLE PERFORMANCE

When performance is determined by the branch leadership to be unacceptable, the contracted instructor will be informed that performance is unacceptable and why, and request his or her initials and date on the progress report or appropriate checklist. By initialing and dating the form, the contracted instructor is acknowledging that he has been told of the unacceptable performance. However, initialing does not necessarily mean the representative agrees performance was unacceptable.

If the contractor agrees to correct the unacceptable performance, corrective measures or re-performance shall begin within one working day from time of agreement. After re-performance by the contractor, branch leadership shall re-evaluate the instructor. If performance is still found to be unacceptable, re-performance and subsequent recheck will be repeated until acceptance is reached. If an acceptable re-performance cannot be met the contractor will be notified and a replacement for the contracted instructor will be required.
