PAST PERFORMANCE QUESTIONNAIRE
Instructions:
· The Offeror will complete Sections II and III (blocks A through C) and provide the form to the assessor (Government personnel who had experience with the Offeror on a previous relevant contract). The Offeror should also submit with its proposal (by the closing date of the Solicitation) copies of Sections I through III of all questionnaires as provided to assessors.
· The assessor, will verify Sections II and III, correcting any information accordingly. The assessor will complete the questionnaire and submit it to the Contracting Office.
Message to the assessor: Your assistance is requested by the US Marine Corps to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Instead, please e-mail or FAX the completed questionnaire(s) to: judith campbell@usmc.mil or fax to 703-784-5908.

| The objective of this requirement is to continue execution of efforts to deploy and support M-SHARP and implement new and enhanced functionality of the application by July 2020 and beyond to fully support the Fleet Marine Forces, Headquarters Marine Corps (HQMC) and Aviation Training System (ATS) needs of Marine aviation, allow more simulator and ground utilization, increase functionality on multiplatform based systems, and increase security and current functionality. M-SHARP application increased functionality and sustainment will allow for ground and other units to use and build on current capabilities. Sustained development and support of a user friendly, web-based and stand-alone application are paramount to ensure M-SHARP remains a viable and authoritative resource for US Marine Corps Aviation flight hour and sortie based training data. |
