PAST PERFORMANCE QUESTIONNAIRE (PPQ)
Instructions:
· Sections I through III to be completed by the Offeror/Major Subcontractor and provided to the assessor (Government personnel who had experience with the Offeror/Major Subcontractor on a previous relevant contract). The assessor, in turn, will verify Sections I through III, making corrections as necessary; complete the questionnaire; and submit it to the Contracting Office at least two (2) days prior to the close of the solicitation on 15 January 2020.
· Sections IV through VII to be completed in their entirety by the assessor.
Message to the assessor: Your assistance is requested by the USMC to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Instead, please e-mail the completed questionnaire(s) to: amanda.dupont@usmc.mil, judith.campbell@usmc.mil, and michael.t.curley@usmc.mil. Questionnaires must be received from the references and not the contractor.

| Project/Requirement Description |
| This requirement is to fabricate and install exhibits for the Final Phase projects in the following two (2) areas: Gallery 13 Forward Deployed and Gallery 14 Iraq and Afghanistan. The Offeror’s responsibilities include: fabrication and installation of all exhibitry platforms and scenic surfaces; interpretive signage and graphic panels, graphic murals, stand-off panels, associated support components, engineering and installation of overhead systems to display graphics and macro artifacts; themed environments, replicated rockwork and vegetation, painted murals, themed tableaus; themed entrances; installation of life-cast figures (provided as Government Furnished Equipment (GFE) from the Government); artifact handling and mounting; overall site coordination of all contractors associated with this project, safety, site cleanliness; display case lighting and exhibit casework. |
