252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (MAY 2013)

(a) Definitions. As used in this clause--

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall--

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through

WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.

(e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:

(1) Document type. The Contractor shall use the following document type(s).

(4) Payment request and supporting documentation. The Contractor shall ensure a payment request includes appropriate contract line item and subline item descriptions of the work performed or supplies delivered, unit price/cost per unit, fee (if applicable), and all relevant back-up documentation, as defined in DFARS Appendix F, (e.g. timesheets) in support of each payment request.

(5) WAWF email notifications. The Contractor shall enter the email address identified below in the “Send Additional Email Notifications” field of WAWF once a document is submitted in the system.

U. S. Department of Labor Wage Determination Number 2005-2473, Revision Number 15 Dated 25 June 2013 is hereby incorporated into this contract and any subsequent order. See attachment.

Section H - Special Contract Requirements

CONTRACTOR MANPOWER REPORTING

The contractor shall report contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the inspection of the Iwo Jima Monument via a secure data collection site. Contracted services excluded from reporting are based on Product Service

Codes (PSCs).

The excluded PSCs are:

(1) W, Lease/Rental of Equipment;

(2) X, Lease/Rental of Facilities;

(3) Y, Construction of Structures and Facilities;

(4) S, Utilities ONLY;

(5) V, Freight and Shipping ONLY.

The contractor is required to completely fill in all required data fields using the following web address https://doncmra.nmci.navy.mil.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar

CONTRACTOR ACCESS TO MCRD PARRIS ISLAND, SOUTH CAROLINA

The Marine Corps Recruit Depot/Eastern Recruiting Region policy allows no weapons or illegal drugs to be brought aboard our military installations. Individuals operating a vehicle aboard our military installations are required to have a valid driver licenses, registration, and insurance.

(a) The contractor shall understand and comply with Depot Order 5512.11 Access Security. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who perform work on the Marine Corps Recruit Depot cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The format for the letter must be in compliance Depot Order 5512.11. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward to the Depot Security Manager Office for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and any additional requirements to comply with local security procedures to obtain a vehicle pass.

(c) The Contract awardee shall be responsible for obtaining required identification for newly assigned personnel. The contractor is required to complete access forms for all employees requiring access aboard the Depot for completion of the background check by the Depot Security Manager and approval by the contracting officer prior to commencing work.

(d) A criminal background check conducted by the Depot Security Managers Office shall be required for all contractor personnel who make frequent visits to or perform work on the Marine Corps Recruit Depot cited in the

Contract, prior to the start date of the contract. Contractors are required to review any proposed employee prior to submitting their names for the background check. Depot access maybe denied if it is determined that the employee(S):

1. Is on the National Terrorist Watch List.

2. Is illegally present in the United States.

3. Is subject to any outstanding warrant.

4. Been issued a debarment order and currently ineligible to enter any military installation.

5. A habitual criminal offender.

6. Been convicted of a felony within the past ten (10) years.

7. Been convicted of a crime involving drugs or drug paraphernalia within the past five (5) years.

8. Knowingly submitted a false of fraudulent information in connection with an employment questionnaire.

9. A registered sex offender.

10. Displays or has displayed conduct that the Commanding General deems prejudicial to the good order and discipline of this installation.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are properly returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1)in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

INSTRUCTIONS TO OFFERORS

1. Solicitation number M0026314T0028 is issued as a Request for Quote (RFQ) using the commercial procedures in accordance with FAR Part 12 and Simplified Acquisition Procedures FAR Part 13. This solicitation is unrestricted under North American Industrial Classification System (NAICS) 541511. The proposed contract is for services for which the Government intends to negotiate with only one source (Harris Integrated Solutions, Inc., 304 Parnell St, West Columbia, SC 29169). See attached Sole Source Justification. All offerors may submit a quote that will be reviewed by the Government. However, a determination by the Government not to compete with this proposed contract based upon responses to this solicitation is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.

2. Any/all questions regarding this RFQ shall be received by this office by August 11, 2014 at 4:30 PM Eastern.

3. Quote Deliverables

A. Offeror shall complete provision, FAR 52.212-3 or complete them in SAMs (System for Award Management) and submit with them with the SF 1449 quote.

B. Offeror shall provide a Performance Work Statement (PWS) or Statement of Work (SOW) that demonstrates the offerors ability to successfully complete the tasks outlined in the attached Statement of Objectives (SOO).

C. The contractor shall complete blocks 30a through 30c on page 1 of the SF 1449 and include the Offeror name, Point of Contact, Cage code, Duns code, and Tax ID #.

D. Offeror must acknowledge all solicitation amendments.

E. Proposals must be good for 45 days from the closing date of the solicitation.

F. Submit three (3) past performance references containing the information requested below as part of the quote. The references shall be relevant contracts completed within the past three (3) years similar in technical scope, complexity and dollar value to this requirement.

Section M - Evaluation Factors for Award

52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Selection for award will be based on price only. Quotes that are non-compliant with any material requirement of this solicitation may be rejected without further consideration for award. The apparent successful offeror shall have “acceptable” past performance. This will be a firm fixed price contract and will be awarded utilizing Simplified Acquisition Procedures.

Past performance will be rated on an “acceptable” or “unacceptable” basis. The past performance evaluation shall consider the successful offeror's demonstrated recent and relevant record of performance in providing services that meet the contract’s requirements. Relevancy shall be similar services in terms of scope and complexity occurring within the past three (3) years. In addition to past performance references, potential sources of performance data are Government sources such as the Past Performance Information Retrieval System (PPIRS) and the Federal Awardee Performance and Integrity Information System (FAPIIS). Past Performance data shall be reviewed and the quality and usefulness determined as it applies to performance confidence.

If an offeror has no relevant past performance history, an offeror must affirmatively state that it possesses no relevant past performance history. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that not meaningful past performance rating can be reasonably assigned, the offeror will not be evaluated favorably or unfavorably on past performance. In these cases the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “Unknown” shall be considered “acceptable”. However, offerors with an “unknown” rating who fail to provide three (3) references or affirmatively address a lack of references shall be considered unacceptable as regarding past performance and will not receive a "Neutral" rating.

(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
